Medical Liability Waiver and Medical Release. By signing this agreement, I the above athlete understand and agree to the following: I acknowledge that participation in cheer and dance activities, including but not limited to practices, performances, and competitions, involves certain inherent risks, including the risk of injury. I voluntarily assume all risks associated with participation and agree that High Voltage All Stars - its coaches and volunteers, event providers, and their staff shall not be liable for any injuries or damages sustained as a result of participation in these activities. In the event that medical attention is required during my participation in cheer and dance activities, I authorise High Voltage All Stars coaches or staff, to contact my emergency contact as listed above, to seek and consent to medical treatment. If unable to contact emergency contact I authorise High Voltage All Stars coaches or staff, on my behalf to seek and consent to any medical treatment deemed necessary under medical guidance. I authorise High Voltage All Stars coaches or staff to provide my medical information as known to enable treatment as required. I understand that I am solely responsible for any costs incurred for medical treatment, including but not limited to ambulance services, and agree that none of the aforementioned parties shall be liable for any such costs. I hereby release, waive, discharge, and covenant not to sue aforementioned parties from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or any property belonging to me while participating in cheer and dance activities. I have read this agreement, understand its contents, and voluntarily agree to its terms.
