Consent to Background Check
By signing below, I authorize Benton Christian Community Church to conduct a background check that may include the following: Criminal history, Employment verification and history, Sex offender registry check, Credit history (if applicable), and References relevant to my qualifications for employment. I understand that the results of the background check will be used to determine my eligibility for employment with Benton Christian Community Church, and that information revealed by the background check may result in my not being hired or may disqualify me from further consideration in the hiring process. I understand that any employment decision based on the results of a background check will be made in compliance with applicable federal and Wisconsin law, including laws governing the use of arrest and conviction records. I understand that I have the right to request a copy of any background report obtained and to dispute any information contained within it, as provided by law.
By signing below, I certify that all information provided in this application is true, complete, and accurate to the best of my knowledge, and that I have not knowingly withheld information that might adversely affect my consideration for employment. I understand that any misrepresentation or omission of facts may result in refusal of hire or termination of employment if hired.
I authorize the investigation and verification of all statements made in this application. The church may contact references, employers, public agencies, and educational institutions to verify information provided by me in this application, my resume, or interviews. As allowed by law, references may provide information concerning my prior employment and other pertinent matters. I release Benton Christian Community Church and all parties involved from liability arising from the furnishing or use of such information, to the extent permitted by law.
I understand that neither the acceptance of this application nor entry into any employment relationship with Benton Christian Community Church creates an actual or implied contract of employment. If hired, my employment will be at-will, meaning that Benton Christian Community Church or I may terminate the employment relationship at any time, for any lawful reason, with or without cause, as permitted by Wisconsin law.
I understand that Benton Christian Community Church does not unlawfully discriminate in employment and operates in compliance with applicable federal and Wisconsin non-discrimination laws.
I further understand that positions involving pastoral or ministerial responsibilities are filled in accordance with the church’s theological convictions and doctrinal standards. As a Christ-centered, Bible-believing church, Benton Christian Community Church reserves the right to make employment decisions on the basis of religion consistent with its faith, mission, and governing documents.
