Hi!! Thank you for checking out my booking form :) Please read all booking policies on my instagram before filling out this form. Fill out this form to the best of your ability. You don't have to know exactly what you want or have a fully formed idea to fill out this form! I'm happy to discuss ideas or schedule a consultation to solidify the design after you've filled out a form :) Once you have submitted, I will respond via email, usually within a few days. ***this form is for appointments in NYC***

Please list a few preferred dates and times for your appointment - in JUNE I will be taking appointments on the following dates: 3, 4, 17, 18, 25, 26, 27, 28, 29, 30. in JULY I will be taking appointments Mon, Tues, Fri, Sat with some more flexibility to open extra dates if necessary.
I will do my best to accommodate your time requests, but please be flexible if possible.

Existing clients that refer new clients get a $50 tattoo credit for every new referral that books an appointment!
Policies

Please read completely!
Once you submit this form I will reach out within the following days via email to let you know if I can take on your request. I will email you with next steps on how to submit your deposit. Submitting a booking form is not a guarantee of an appointment.
Once a deposit is submitted, you will receive a second email with a link to select your appointment time and lots of info about your appointment.
A $50 *non refundable* deposit is required in order to secure an appointment. This deposit will go toward the final payment at the end of your session.
I ask that any reschedule requests be made at least 48 hours in advance. Your deposit is good for one reschedule made within 24 hours of the reschedule request. Please be aware I may not be able to accommodate rescheduled appointments right away but will try my best. If there is a last minute cancellation or reschedule, a new deposit will be required to reschedule another appointment, and this deposit will not be applied to the final tattoo cost. No-call no-shows will not be allowed to rebook in the future.
An additional $50 drawing fee is required for custom designs. This fee covers 2-3 versions of your design. If more versions and alterations are requested an additional $50 fee will be required for 2 more versions
Tattoos are priced by piece starting at $200 (usually averaging $300-$450). The cost of your piece depends on the pieces size, placement, detail, and estimated time to complete. Once we have discussed your idea I am happy to give an estimate or range for the cost of your tattoo. I AM ALWAYS WILLING TO WORK WITH YOUR BUDGET! I’m happy to figure out a payment plan if necessary :)
The price you are quoted for your tattoo may vary depending on the actual length of the appointment. Please keep in mind that the amount that I quote you for this piece is a starting price and may vary
Please let me know if you are running late - you can DM me on instagram @paxhandpokes, or @giftshop.tattoo. If you are more than 30 minutes late your your appointment we may have to reschedule and another deposit may be required
Tattoo appointments are usually no less than two hours, and can vary greatly depending on sizing, placement, and design. More detailed designs take 5-8+ hours. Some flash or custom designs may take multiple sessions to complete - please plan accordingly when booking!

Studio Info
We have a studio dog belonging to one of our residents that is around sometimes, please let me know if you would prefer to not have a dog in the studio during your appointment.
We currently have a no-guest policy (most of the time). We have a small space and prioritize the comfortability of our clients and residents.
