    These are additional emergency contacts, beyond parents. List the name of at least one person who can be contacted in the event of an emergency or illness if you cannot be reached. Any person listed should be able to assist in contacting you. At least one person listed must be within one hour of the center/home, able to take responsibility for the child in case the parent/guardian cannot be contacted and should be at least 18 years of age. The first emergency contact must live no more than 1 hour away and be over the age of 18.

  • Red Clover Camp Parental Consent Forms

  • Release of Liability

    I understand that my child’s participation in activities with the Red Clover Camp program (“RCC"), including but not limited to hiking, biking, swimming, and the like, can expose my child to dangers from known and inherent risks, as well as unanticipated risks. Acknowledging that such risks exist, I hereby release and discharge Red Clover Camp, and its officers, agents, and employees, from any and all liability from injuries, claims, loss of services, expenses and/or damages that may be sustained by my minor children on account of his/her participation in said programs or associated activities.

  • Emergency Transportation Policy

    Red Clover Camp has permission to secure emergency transportation for my child in the event of an illness or injury which requires emergency treatment. The emergency transportation service will determine the facility to which my child will be transported.

  • Cancellation and Schedule Adjustment Policy

    A camper will not be considered registered until all required registration fees and tuition are paid.

    Red Clover Camp requires a non-refundable registration fee of $125.00 (new) or $100 (returning) per student for summer camp and tuition is required to be paid in full at the time of registration. 

    Registration cancellations and changes requested between January 20th and March 31st will result in a loss of paid registration fee and are subject to availability.

    Registration cancellations and changes requested between January 20th and March 31st may be refunded. All refunds will be less transaction fee assessed by Paypal or Venmo. All approved refunds will be processed by at least 7 days prior to week 1 of summer camp.

    As of April 1st registration cancellations and changes are not eligible for refund of either registration fee, tuition paid, or tuition due.

    If adding summer camp weeks after April 1st tuition must be paid for independently and previously paid tuition for cancelled weeks may not be applied to this transaction.

    Existing students of Mountain Village School may request to pay tuition on a weekly basis but registration fee is still due at time of registration. If electing to pay tuition on a weekly basis you are still required to adhere to the Red Clover Camp Cancellation and Schedule Adjustment Policy.

    Weekly tuition will be processed every Thursday. If weekly tuition payment is declined or rejected you may forfeit any paid deposit(s), camp fees and your child’s spot in camp.  Late payments will be assessed a $25 processing fee per week, per student. 

    All requests for cancellations and changes, as well as inquiries for financial aid, should be submitted to the Financial Director, Nick Crandall, at: office@redclovercamp.com  and are subject to availability.

  • Family Handbook and Camp Expectations

    * I understand that the RCC Handbook can be viewed and downloaded at www.redclovercamp.com, and I acknowledge that RCC has met its obligation to inform me of its policies and procedures by providing me with the Handbook. I understand that if I have a question regarding a specific area of content in the Handbook, the Director will clarify the area for me.

    * I agree to follow all program policies as stated in the Parent Handbook.

    * I am aware that certain activities are physically demanding. (If you have concerns regarding your child’s abilities to participate fully, please contact the Camp Director.) I have noted that the most activities are conducted outside, in all kinds of weather. I will send my child to camp with the appropriate equipment, such as rain gear, sturdy shoes, protective clothing, sunscreen, water bottles, etc. I further understand that equipment, clothing, backpacks, lunchboxes, etc. will experience wear and tear. I understand that both my child and their belongings may come home wet, dirty, muddy, and tired.

    * I understand that money, video games, toys, electronic devices, cell phones, trading cards, jewelry, and personal sporting equipment are not to be brought to camp. I understand that RCC is not responsible for their loss or misplacement.

    * I understand that clothing, towels, shoes, coolers, swimsuits, and water bottles will be marked with the child’s name. I understand RCC is not responsible for their loss or misplacement.

    * I understand that my child may not be released to anyone without prior written documentation and presentation of a valid photo identification.

    * I understand that RCC staff cannot withhold a child from a biological or custodial parent without legal documentation (i.e. court orders, custody papers, etc.)

    * I understand my child will be sent home from Camp if he/she demonstrates inappropriate or aggressive behavior. If behaviors are continuous and uncontrollable or put others at risk my child may be asked to leave camp. No tuition refunds will be granted if a child is sent home or expelled from camp due to behavior.

    * I understand that any and all prescription medicine to be administered by camp staff must be sent to camp in a current prescription bottle. I understand that over the counter medications must be sent in their original container. I understand that camp staff may only administer medication as it is prescribed on the proper container. I understand that medication will be sent home with the child on a daily basis.

    * I understand that sunscreen sent to camp must be given to staff and will remain at camp for duration of the summer or until the Friday of the child’s last week of summer camp.

    * I understand some specific hazards that may be encountered during the course of a normal camping day may include: slipping or falling, insect bites, ticks, poison ivy, or bodily injury. I understand and recognize the element of risk inherent in any adventure program, sport, or activity associated with the outdoors. I certify that my child is capable of participation in all of the daily activities and I agree to reinforce the established safety rules and procedures.

    * I understand that credits will not be issued for any absences or for program closings due to severe weather or other facility closings where other arrangements have not or could not be made by RCC.

    * I understand that the RCC is not responsible for and discourages employees from providing paid care and custody for a camp participant under the age of 18 outside of an RCC Program (i.e. babysitting).

    * The information given in the registration and medical information paperwork is correct and complete to the best of my knowledge and the person herein described has permission to engage in all activities in the program, except as noted (all necessary and important medical information regarding my child has been documented on the medical forms provided in the registration paperwork).

    * I understand that there will be a lifeguard on duty when my child is swimming and will be asked to perform a swim test if desired to enter the deep end of the body of water.

    * I give my permission for my child to take part in the field trips provided by the Camp program. I understand that my child will walk, or be transported by our MVS busses.

    * I understand RCC staff may photograph or video record my child for RCC promotional or onsite activity purposes.

  • Red Clover Camp Week Section

    Each week of camp consists of field trips to local parks, bike trips, arts and crafts, pool time, field games, and river play. We add in additional activities for longer bike trips and field trips as the summer goes. 
  • Billing Information and Details

    No payment will be collected when this form is submitted. All billing will occur through Tuition Express as detailed below. Registration is not confirmed until camp balance has been successfully processed.

    • Nick or Ali will be reaching out if we don't have an up-to-date payment method on file for your family within Tuition Express.

    • You'll be asked to provide checking account or credit card information through a secure link via Tuition Express, to be billed for the registration amount due. PLEASE MAKE SURE TO COMPLETE WITHIN FOUR DAYS OF RECEIVING THE LINK. 

    • The balance due will be processed within two weeks of receipt of registration.

    • Current and past MVS and RCC families should select their preferred payment option below.
