    Please note: It is the parent/guardian's responsibility to ensure that all necessary documnentation is received during the designated application period. An application is not considered complete without the required student records being turned in to the Elementary office.

  • If yes, you will need to bring in their current IEP/ETR to the Elementary office.

  • Is the child currently receiving accommodations on a 504 plan?*

  • If yes, you will need to bring in a current 504 to the Elementary office. 

  • Has the child been identified as gifted?*

  • If yes, and the child has a WEP/WAP, you will need to bring it in to the Elementary office. 

  • Has the child been suspended or expelled from school during the current or previous year?*

  • By submitting this form, you understand that submission does not mean approval. In order to complete the application, the following forms must be brought to the Elementary office

    • A recent official transcript or record of achievement (grade card)
    • birth certificate and social security card
    • a record of any suspensions or expulsions from the currently and/or previous school term (if applicable)
    • current IEP, 504, WEP/WAP (if applicable)
    • copy of parent ID
