Liability Waiver and Parental Consent Form for Oregon Athletic Growth LLC Personal Training

I, the undersigned participant, acknowledge that I have voluntarily chosen to participate in personal training sessions provided by Oregon Athletic Growth LLC. I understand that participation in physical exercise and training involves inherent risks including, but not limited to, injury, illness, or death. I hereby assume all risks associated with my participation and agree to release, waive, discharge, and hold harmless Oregon Athletic Growth LLC, its trainers, employees, and agents from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by me during or as a result of my participation.

As the parent or legal guardian of the minor participant named above, I hereby consent to their participation in personal training sessions with Oregon Athletic Growth LLC. I acknowledge and understand the risks involved and agree to the terms of the liability waiver on behalf of the minor. I also agree to release and hold harmless Oregon Athletic Growth LLC and its representatives from any claims arising from the minor's participation.

I certify that I have disclosed all relevant medical conditions, injuries, or health concerns to Oregon Athletic Growth LLC prior to participation. I understand that it is my responsibility to inform the trainers of any changes to my health status that may affect my ability to safely participate in training sessions.
