  • JOB SUMMARY
    The Marketing & Community Outreach Liaison is responsible for driving agency growth by developing and maintaining referral relationships that generate both Private Pay patients and insurance-based referrals. This role focuses on community engagement, healthcare partnership development, and strategic marketing initiatives aligned with Cross Cultural CareLink’s mission of delivering culturally sensitive, high-quality home health services. 


    This position requires a skilled marketer and relationship-driven professional who can represent the agency with credibility, compassion, and consistency while meeting referral and growth targets. 

    Reports to: Chief Operating Officer

    Salary Range: $70,000 – $120,000 annually, commensurate with experience, performance, and referral outcomes.


    QUALIFICATIONS

    • Bachelor’s degree preferred (Marketing, Business, Healthcare Administration, or related field) 
    • Minimum of 1–3 years of marketing, sales, or outreach experience, preferably in home health, healthcare, or human services 
    • Demonstrated success in referral development and relationship-based sales 
    • Knowledge of Medicaid/MassHealth, private pay services, and community healthcare resources preferred 
    • Strong sales, marketing, interpersonal, communication, and presentation skills 
      Ability to work independently and meet referral targets 
    • Must pass a criminal background check 
    • Reliable transportation required 

    RESPONSIBILITIES/ESSENTIAL FUNCTIONS

    • Build and maintain strong referral relationships with hospitals, assisted living facilities, long-term care facilities, outpatient clinics, physician practices, senior centers, community organizations, and faith-based institutions. 
    • Actively market agency services to generate both Private Pay and Insurance paid referrals. 
    • Conduct in-person visits, presentations, and follow-ups with referral sources. 
    • Represent Cross Cultural CareLink at community events, health fairs, and networking opportunities. 
    • Analyze past and current marketing data to identify trends, opportunities, and gaps. 
    • Assess service area potential and identify high-impact target markets. 
    • Develop and implement a structured marketing and outreach plan with clear priorities and timelines. 
    • Track and report referral activity, conversion rates, and outcomes. 
    • Maintain a monthly referral quota of unduplicated 10 new patients  
    • Strengthen Cross Cultural CareLink’s brand as a trusted, culturally sensitive home health provider. 
    • Educate referral partners on agency services, payer sources, and patient eligibility. 
    • Collaborate with intake, scheduling, and clinical leadership to ensure smooth referral-to-admission workflows. 
    • Gather feedback from referral sources and patients to support continuous improvement. 
    • Maintain accurate documentation of outreach activities, referral contacts, and outcomes. 
    • Participate in leadership meetings and provide regular updates on marketing performance. 
    • Adjust strategies based on data, leadership direction, and agency goals. 
    • Perform other duties as assigned to support agency growth.

    WORK ENVIRONMENT

    • Office-based administrative work combined with regular field travel for outreach and meetings 
    • Community-based settings including healthcare facilities and partner organizations 

    FUNCTIONAL ABILITIES

    • Ability to hear, speak, read, and communicate effectively in English. 
    • Ability to present information clearly to diverse audiences. 
    • Ability to travel independently to referral sites and community locations. 

    PHYSICAL REQUIREMENTS

    • Ability to sit, stand, walk, and drive for extended periods. 
    • Ability to lift up to 20 lbs (marketing materials, supplies). 
    • Ability to work flexible hours as needed for community events and meetings.
