    In order to secure your spot for upcoming surgery support group sessions, please fill in the information below.
  • Foria Clinic is piloting free virtual support groups to help individuals prepare for surgery, understand procedures and expectations, and plan for post-operative care. These groups will be run by a nurse practitioner and a facilitator with lived experience. Registration is open to Ontarians aged 16 and over. 

    This brief (5-10 minute) confidential form will help us assess community interest and shape future programming to best meet participant needs.

    Pilot Program Details
    Session dates will be announced once groups are finalized 
    2-3 90 minute long sessions run on a weekly basis, divided based on support needs
    Free of charge 

    Completing this registration shows your interest in participating in the Surgery Support Groups. Those who register will receive updates on our programming as they become available. You don't need to be a Foria patient to register.
