    Please use the name, email, and phone number of the 18+ adult who will be picking up the camper's complimentary park pass! Photo ID is required at pickup!

  • O-Town Hockey Camp Refund/Cancellation Policy

    Please know that our cancellation policy is in place because we prepare each individual camp, with the projected number of sign-ups. Our overhead costs, with staffing, ice, travel, etc. are based on the number of campers signed up for that particular camp, which serves as the basis for our cancellation policy.

    • Cancellations or refund requests made before May 20, 2026 will receive a $250 refund.
    • Cancellations or refund requests made after May 20, 2026, are not eligible for any refund or credits. There are no exceptions. We understand things happen, but credits and refunds will not be processed after May 20, 2026.
    • No refunds or credits will be issued due to late arrival or early departure, voluntary withdrawal, no-shows, or expulsion from camp. No refund or credit will be given to any camp participant who fails to attend the camp/clinic without notification to O-Town Hockey Camp.
    • Your full payment for, and participation in, the O-Town Hockey Camp is a non-negotiable requirement for receiving free and/or discounted Universal Studios Park Tickets and/or accompanying hotel discounts associated with participating in the O-Town Hockey Camp. Any refund request or cancellation of participation in the O-Town Hockey Camp will render any FREE or discounted Universal Studios Park Tickets or hotel accommodations null and void.
    • No refunds or credits will be given for participants who check-in and who do not finish camp unless it is injury or sickness related to official medical documentation.
    • If a camper becomes sick or injured at a camp/clinic or before a camp/clinic, which prevents them from being able to participate, a credit may be issued for a future camp/clinic. Official medical documentation is required.
    • O-Town Hockey Camp will not provide refunds for cancellations due to unforeseen acts of God, pandemics, or man-made disasters, including facility damage/closing complications or weather-related rink/facility cancellations. These are highly unlikely to occur, but in the event it does, we will issue credit vouchers for the amount paid, or prorated for the remainder of the affected program.
    • If O-Town Hockey Camp cancels for any other reason not specified above we will offer you a full refund of the camp tuition. O-Town Hockey Camp will not refund for incidentals arising from the cancellation of a camp such as hotels, amusement park costs, other travel costs, etc.
    • O-Town Hockey Camp reserves the right to cancel or change camps/clinics, dates, times or locations.
    • Throughout camp, all participants are expected to behave professionally, to adhere to all camp guidelines and policies, and to treat their fellow participants, facility staff and O-Town Hockey Camp staff with respect. Participants are expected to represent themselves and O-Town Hockey Camp with integrity, maturity and behavior consistent with achieving excellence. Thank you in advance for being a 'professional' and for being a huge part of ensuring an amazing experience for all.
    • Behavior contrary to the above will be dealt with swiftly, not excluding expulsion without refund.
    • O-Town Hockey Camp has a zero-tolerance policy with respect to uncontrollable behavior, bullying, hazing, alcohol, tobacco, drugs, and controlled substances, weapons of any kind, or any other activity which we deem illegal or not in conformance with our standards. If a camper is expelled due to any of the above circumstances, no refunds will be given.
    • Any requests for a refund or credit must be submitted in an email to otownhockeycamp@gmail.com. Requests given to staff members or phone requests will not be considered. By proceeding, you hereby agree and consent to all of the terms of this Cancellation/Refund policy.
  • O-Town Waiver and Release from Liability

    I understand that my son or daughter will be participating in activities that involve serious risk of injury, including permanent disability and severe social and economic losses, which might result from their own actions, from the actions of others, from the rule of play, or from the condition of the competition area and/or the related facilities or equipment used in the activities in which they will be participating. I also understand that there may be other risks not known to me or not reasonably foreseeable at this time. I assume all of the forgoing risks and accept personal responsibility for any resulting injury, permanent disability or social and economic losses. I hereby release, waive and discharge OTHKY LLC D/B/A "O-Town Hockey Camp" composed of Matt Schwartz, Christian Hanson, Michael Sproules and their respective employees and representatives, including but not limited to Christian Hanson and Michael Sprouls, (collectively the "released parties") from, and agree not to sue any such released parties for, any claims demands, losses or damages which result on account of any injury, permanent disability or social or economic loss caused by, or alleged to have been caused by, in whole or part, any cause whatsoever, except for the sole negligence of any such released party. I have read this Waiver and Release from Liability and understand that I have given up substantial rights by agreeing to it. I have agreed to this Waiver and Release from Liability voluntarily, intending to be legally bound, and understand that my execution of this document is an express requirement for my son or daughter's participation in the O-Town Hockey Camp. This waiver and release shall remain in effect for all activities in which he or she participates on or after the date that I have executed this form. By checking the box, below, you acknowledge that you have read this Waiver and Release from Liability and agree to its terms.

  • O-Town Media Waiver

    I hereby consent and agree that OTHKY LLC D/B/A "O-Town Hockey Camp" composed of Matt Schwartz, Christian Hanson, Michael Sproules and their respective employees and representatives, including but not limited to Christian Hanson and Michael Sprouls, have the right to take photographs, videotape, or digital recordings of my son or daughter while participating in the O-Town Hockey Camp, and to use these in any and all media, now and hereafter known, for promotional purposes, to show video highlights to be used on www.otownhockey.com and for various other uses on our social media outlets (Facebook, Twitter, Youtube, Instagram, etc.) and online accounts. I do hereby release to Quest Hockey all rights to exhibit this work in print or electronic form publicly or privately; I waive any rights, claims, or interest I may have to control the use of my son or daughter's likeness in whatever media is used; I understand that there will be no financial or other remuneration for recording my son or daughter, either for initial or subsequent transmission or playback.
