Once completed, it does not need to be filled out again if your child attends further sessions unless your child's information has changed and needs to be updated.

Payments should be made on or by the Thursday before your child attends their first Saturday session to: Myelle Peralto || NCB Constant Spring Branch || Savings account #334463664
Payments should be made via bank transfer and a screenshot of the payment should be sent VIA EMAIL to thesensoryspaceja@gmail.com. After, a receipt will be sent to your email!

The Hands-On option below is ONLY for parents of children who have special needs or require more one-on-one assistance from one of our aunties AND children under 2 1/2years old for their FIRST session with us.

Please note that if your child is unable to attend the session that has been signed up for (due solely to illness or unforeseen circumstances) and payment has already been made, it will be credited up to 2 months after the date of the session only if we receive notice from you before the time of the session. Sessions with HOF option may be credited in full if notified by the Thursday before the session, and in half before the time of the session on Saturday.

FIRST Saturday Session + Hands-On Fee
Register for one session only + Hands-On assistance. Applicable to ALL children under 2 1/2 years old. We assign one auntie to your child for the FIRST day that they attend a session with us so that they can get acquainted with us and the space as we understand it is a new experience for them. After the first session, they are applicable to the standard $5,000/session unless they will require continuous hands-on (children with special needs, autism etc.).

This agreement is subject to change based on the health regulations set by both the venue (Rainbow Land Nursery School and Pre-K) and Jamaica's government mandates
I, the parent/guardian of this child understand that the organizers and/or staff will have made every effort to keep my child safe and involved during their stay at the camp. I understand that I am not contracting with Rainbow Land and that the camp is not a program run by Rainbow Land. I give authority to the staff to apply judgement in regards to medical assistance in the event of an accident, injury, or illness IF the parent/guardian or emergency contact person(s) cannot be reached. I authorize any first aid and treatment which may be deemed necessary. I release Rainbow Land Nursery & Pre-K and The Sensory Space JA staff of responsibility in case of accident, illness, or injury during my child's enrollment.

Health Details:

▪ Upon arrival, parents and children must wash their hands at the hand washing station.

▪ If a child has a fever or signs of sickness (i.e.: runny nose, chest congestion etc.) they will not be permitted to enter. If a child becomes sick or shows signs of sickness during their time at a session, the child will be gently removed from the space and a parent/guardian will be called to pick up the sick child immediately.

▪ If a child or parent has contracted Covid-19 or any virus (i.e.: the Hand, Foot and Mouth Virus etc.) during the time of camp or within 5 days after camp, we will need to be notified so that we can make our necessary checks.

▪ At drop-off time a parent/guardian can walk his/her child to the entrance to say goodbye as we know that the separation process is at times difficult for some children and for some parents too. We will do our best to keep you updated! ♥️
