Please note filling out this form, DOES NOT gurantee that you will be picked. If you are chosen your design, must go along with the theme of what you are raising money for. Our Logo will be included on the back upper area of the shirt. If you are chosen you will need to order all of the shirts at the same time. You will only be able to order once at these prices.

You will get one print area with this for this and it will need to be a min. of 30 shirts.

Sizes S-XL - $8 per shirt
Sizes 2XL and up - $10 per shirt

You will be able to choose the color and sizes that you would like to get.

These forms will be open til January 22nd, and we will make a decision by the 27th. If you are picked, you need to have your order ready for us by Feb 2nd.

If you are not chosen and we open it up again, you will need to refill out any forms.
