Ireland FutureVet Camp 2026 Registration Form

If you are 16 or older

There will be 3 x ONE day FutureVet camps in Ireland.

Two of the camps will be on the 29th and 30th October at Mountbellew College. There will be one camp at Kildalton College on 20th August.

Please choose your first and second choice of date and venue which you prefer.

Cost €70* per student including lunch, three practical sessions, one clinical skills session, talks from two Irish Vet Schools admissions teams plus an opportunity to meet vets and vet nurses.

Consent to Use Photographs and Video Footage Future Vet Camp

FutureVets (With the Waves Limited) may take photographs and/or video recordings of participants during FutureVet camp. These may be used for:

Promotional materials (print and digital)
Social media platforms
Websites
Press or media coverage
Reports or training
Printed promotional materials

I, the undersigned, confirm that:
I give permission for photographs and video footage to be taken of me during the veterinary camp.
I understand that these may be used for publicity, promotional, or educational purposes by FutureVets (With the Waves Limited) and associated organisations.
I understand that these images may appear in printed or digital formats, including social media.
I understand that this consent is voluntary and that I may withdraw it at any time by contacting the camp organisers.
I understand that withdrawal does not apply to materials already published.

Data Protection Statement:

All personal data (including images) will be handled in accordance with the UK GDPR and the Data Protection Act 2018. Images and video will be stored securely and only used for the purposes outlined above.

Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by FutureVets during the selected camp. In exchange for the acceptance of said child’s application by FutureVets, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless FutureVets, and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against FutureVets including all coaches and affiliates, all participants, sponsoring companies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all activities.

I acknowledge that i and my child have both read , signed and understand the Camp code of conduct including that alcohol, drugs and sexual acts are prohibited at the camp and i hereby acknowledge that if my child breaks the code of conduct they will be immediately suspended from any further participation in the camp, no refund will be given.

Medical Release and Authorization

As Parent and/or Guardian of the named child, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.

Permission is hereby granted to the attending doctor to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named child. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending doctor to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.

Permission is also granted to FutureVets and its affiliates including volunteer staff, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility.

This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.
