Authorization Details
This form gives authorization to Mid Michigan College Financial Aid to credit a late disbursement you are entitled to due to your withdrawal from the College. An email was sent to your Mid Mich Email account indicating the type and amount of aid you are eligible to receive. If you have any questions about the disbursement process, contact Financial Aid.

Please read the information carefully and sign the form.

If you do not wish to accept the funds, email finaid@midmich.edu and we will cancel the award.

I would like to receive the financial aid that I am entitled to in a late disbursement. I understand that these funds will be disbursed directly to my student account. Any overpayment to the account will be sent in a refund check.

If the funds do not pay off the balance on my bill, I understand that I must contact Business Services in order to pay my balance due.

Any loan funds that I receive will need to be repaid to the federal government based on the terms of the promissory note.
