Personal Training & Liability Waiver
Team Roe & Friday Fit
Acknowledgment of Risk:
By participating in any personal training sessions, group fitness classes, boot camps, or physical activities offered by Team Roe and Friday Fit, I acknowledge that participation involves inherent risks, including but not limited to muscle strains, joint injuries, dizziness, cardiovascular complications, or other physical injuries.
Medical Responsibility:
I certify that I (or my minor child) am physically able to participate in all training activities and have consulted a physician if necessary. I understand it is my responsibility to disclose any medical conditions, injuries, or limitations prior to participation.
Assumption of Responsibility:
I voluntarily assume full responsibility for any risks, injuries, or damages that may occur as a result of participation in any training or fitness activities.
Release & Waiver of Liability:
I hereby release, waive, and hold harmless Team Roe, Friday Fit, their owners, trainers, contractors, volunteers, and affiliates from any and all claims, demands, injuries, damages, or liabilities arising out of or related to participation, including those caused by negligence, to the fullest extent permitted by law.
Indemnification:
I agree to indemnify and hold harmless Team Roe and Friday Fit from any claims, damages, or expenses, including legal fees, arising from participation or conduct.
Photo & Media Release:
I grant permission for Team Roe and Friday Fit to use photos or videos taken during training sessions for marketing, promotional, and social media purposes unless I opt out in writing.
Minor Participant & Parent/Guardian Consent:
If the participant is under the age of 18, I certify that I am the parent or legal guardian of the minor participant and I consent to their participation in all training and fitness activities. I agree to all terms of this waiver on behalf of myself and the minor, and I assume full responsibility for the minor’s participation.
Acknowledgment:
By participating or allowing a minor to participate in services provided by Team Roe and Friday Fit, I acknowledge that I have read, understood, and agree to this waiver, and that my electronic acceptance constitutes a legally binding signature.
