It is the policy of Spirit of Joy Preschool to provide emergency care to students as needed while in attendance at school or at school events and to release information requested in connection with the provision of such care. Responsibility for payment of ambulance, physician and/or hospital expenses is that of the parent.

By signing this form, I grant Spirit of Joy Preschool permission:

1. For my child to use all play equipment and to participate in all activities at the school.
2. For my child to leave the premises under the supervision of a staff member for neighborhood walks or special field trips in licensed and insured vehicles. Advance notice of field trips using vehicles will be given.
3. For the teacher or acting teacher to take whatever steps necessary to obtain medical care as warranted.

Spirit of Joy Preschool asks permission to use of picture/video of my child involved in activities at the preschool or on field trips. These pictures could be displayed in printed brochures and materials, on our website, in the church, or as a part of social media. No names will be included.
