I hereby give consent to II Inked, LLC(, referenced as Studio hereafter) to perform a Body Crafting Service, & in consideration of doing so, I release studio and Body Artist from all manner of liabilities, claims, actions, & demands in law, or in equity, which I or my heirs might now or hereafter by reason of complying with my request of such service. The studio has given me the full opportunity to ask any & all questions about the application of my modification & all of my questions have been answered to my total satisfaction. I have been fully informed & understand that risks, known & unknown, can lead to injury. Having been informed of the potential risks associated, I still wish to proceed with said body art application & I freely accept & expressly assume any & all risks that may arise. I am not under the influence of alcohol or drugs, & I am voluntarily submitting to such permeant change by the Artist without duress or coercion.

Body piercings are generally safe when performed by a professional and cared for properly; however, risks do exist. Common complications include pain, redness, swelling, bleeding, or minor infection. More serious issues such as severe infection, excessive bleeding, or nerve damage can occur but are rare. Individuals with diabetes, bleeding disorders, allergies to metals, or weakened immune systems may have a higher risk of complications. By signing below, I acknowledge that I understand these risks and have had the opportunity to ask questions before proceeding. Some individuals experience prolonged healing, scarring, keloid formation, or jewelry rejection, where the body pushes the jewelry out over time.

I release all rights to any photographs taken of me & the service & give consent in advance to their reproduction in print or electronic form. I also release that the design is owned by Studio. If you DO NOT wish for your artist to distribute images you may remind your artist via text however one is needed for archival purposes! Or initial & date here

AFTERCARE ACKLNOWLEDGEMENT

Studio has given written & verbal instructions on the care of my piercing while it's healing, & I understand them & will follow them. I acknowledge if I do not follow the instructions given to me, I acknowledge that it is possible that the piercing can become infected, If you see the first signs of infection (increased redness, swelling, pus, fever), contact a healthcare professional. If any follow-up is needed due to my own negligence, I agree that the work will be done at my own expense.

I hereby declare that I am of legal age (& have provided valid proof of age above) & am competent to sign this Agreement. I agree to reimburse the Artist & Studio for any attorney fees & costs incurred in any legal action I bring against either the Artist or Studio & in which either the Artist or the Studio is the prevailing party.
