  • About this form...

    This form has 2 pages.

This first page contains questions to get you registered for our event.

Page two contains a media release form. It has one yes/no multiple choice question, and a signature request.

TLDR: There's not much to fill out on page two! This page had the most questions.

  • Media Release

I understand that photos, videos and audio recordings of participants will be taken during the event. I also understand that Braided Seeds would like the opportunity to share this media featuring participants for education/field-building and/or marketing efforts for Braided Seeds but will only do so if I grant a full media release. I am also aware that Braided Seeds creates private photo albums to share media amongst staff through Google Photos. I agree to only publicly share media from the private albums that solely feature me and participants registered in this form or I will seek explicit permission from the other participants featured in media I wish to share outside of the private albums.
