SOCCER CAMP TERMS & CONDITIONS
By registering a participant for the soccer camp, the parent or legal guardian agrees to the following Terms & Conditions:
1. Eligibility & Participation
Participants must meet the age requirements set by the soccer camp and be physically able to participate in soccer-related activities. The camp reserves the right to deny participation if a participant does not meet eligibility or safety requirements.
2. Assumption of Risk
Participation in soccer activities involves inherent risks, including physical injury, weather-related conditions, and other unforeseeable risks. The parent/guardian voluntarily assumes all risks associated with participation.
3. Medical Responsibility
The parent/guardian certifies that the participant is in good health and has no medical conditions that would prevent safe participation, or that all relevant medical information has been disclosed. The parent/guardian is responsible for all medical expenses incurred.
4. Emergency Medical Authorization
In the event of an emergency, the soccer camp staff is authorized to seek medical treatment for the participant if the parent/guardian cannot be reached.
5. Behavior & Conduct
Participants are expected to follow all camp rules and instructions. The soccer camp reserves the right to remove any participant for disruptive, unsafe, or inappropriate behavior. Removal due to misconduct does not entitle the participant to a refund.
6. Refund & Cancellation Policy
Registration fees are non-refundable, unless otherwise stated by the camp.
The camp reserves the right to cancel or reschedule sessions due to weather, low enrollment, or unforeseen circumstances.
If the camp is canceled by the organizer, a refund or credit may be issued at the camp’s discretion.
7. Attendance & Late Pick-Up
Parents/guardians are responsible for timely drop-off and pick-up. Repeated late pick-ups may result in removal from the camp without refund.
8. Photography & Media Release
The parent/guardian grants permission for photographs or videos of the participant to be used for promotional, educational, or marketing purposes unless explicitly declined in writing.
9. Personal Property
The soccer camp is not responsible for lost, stolen, or damaged personal items brought to the camp.
10. Changes to Terms
The soccer camp reserves the right to modify these Terms & Conditions at any time. Any updates will apply to all participants.
11. Agreement
By submitting this form, the parent or legal guardian acknowledges that they have read, understood, and agree to these Terms & Conditions in full.
