Maternal & Child Health (MCH) Priority Selection Stakeholder Survey
FFY 2027-2031
Purpose of Survey:
Vernon County Health Department is selecting a Priority Health Issue for the 2027-2031 Maternal & Child Health (MCH) contract. Your responses will help identify needs that are most urgent, feasible, and aligned with families in our community.
SECTION A — Review of Priority Health Issues (With Definitions)
Please review the Priority Health Issues below before answering the survey questions.
Ensure access to patient-centered, coordinated, & comprehensive postpartum care. Supporting birthing people after delivery with coordinated medical care, mental health support, and follow-up through the first year postpartum.
Promote preventive oral health care services during pregnancy. Helping pregnant individuals access dental cleanings, screenings, education, and referrals to dental providers.
Promote safe infant sleep practices and environments to reduce sleep-related infant deaths. Promoting safe sleep education and environments (back to sleep, separate sleep surface) and reducing risk of sleep-related infant deaths.
Enhance access to holistic oral health care services for children. Ensuring children have access to ongoing preventive dental care, including screenings, fluoride varnish, and a dental home.
Ensure coordinated, comprehensive, and ongoing health care services for children with and without special health care needs. Supporting coordinated care across schools, providers, and community services to improve outcomes for children and families.
Promote stable and supportive relationships with a caring, non-parental adult to enhance adolescent psychological well-being and empower youth with the tools and training to reach their full potential. Helping adolescents connect with trusted adults outside the home to support resilience, belonging, and mental well-being.
Promote a smooth and successful transition from child-centered to adult-oriented health care, promoting continuity of care, improving health outcomes, and empowering youth to manage their own health. Supporting teens as they move from pediatric to adult healthcare by improving self- advocacy, continuity of care, and provider connections.
Promote strengths-based services and support to promote healthy family relationships and functioning, enhance resilience, foster social connections, and support children's social and emotional development. Strengthening families through programs that build coping skills, social-emotional development, parenting skills, and support systems.
