    A separate form must be completed for each individual, including minors. For individuals under 18, a parent or legal guardian must complete the required consent and signature sections.

  • I understand that visiting High Horses Center for Equine-Assisted Services (High Horses) and participating in activities on its property may involve inherent risks, including risks associated with horses, other animals, equipment, facilities, terrain, weather conditions, and the activities taking place.

    I understand that horses are large animals whose behavior can be unpredictable and that they may react suddenly to people, sounds, movements, or other circumstances. Potential risks include, but are not limited to, bites, kicks, falls, being stepped on, and other injuries.

    I acknowledge these risks and understand that injuries may occur even when reasonable safety precautions are taken. I agree to follow all posted rules and instructions provided by High Horses personnel and to remain within authorized areas of the property.

    By signing this form, I acknowledge and accept the inherent risks associated with my visit or, if signing as a parent or legal guardian, the visit of the minor identified in this form.

  • Vermont Equine Activities Law

  • WARNING


    Under Vermont Law, an equine activity sponsor is not liable for an injury to, or the death of, a participant in equine activities resulting from the inherent risks of equine activities that are obvious and necessary, pursuant to 12 V.S.A. § 1039. (Added 1995, No. 136 (Adj. Sess.), § 2.)

    The term "equine activity sponsor" includes the HHCEAS, its employees, volunteers, instructors, therapists, contractors, agents and members of its board of directors.

  • Liability Release

  • In consideration of being permitted to enter the property of High Horses Center for Equine-Assisted Services ("High Horses") and participate in any authorized activities, I acknowledge the risks described in this form and voluntarily assume those risks on my own behalf or, if applicable, on behalf of the minor identified in this release.

    To the fullest extent permitted by Vermont law, I agree to release and hold harmless High Horses, its directors, officers, employees, volunteers, and representatives from claims for injury, loss, or damage arising from the inherent risks of equine activities and other ordinary hazards associated with visiting or participating in authorized activities on the property.

    I understand that this release does not waive any rights or claims that cannot lawfully be waived.

    If signing as a parent or legal guardian, I authorize the minor identified in this form to visit High Horses and participate in activities expressly permitted by High Horses personnel. I acknowledge the risks associated with those activities and consent to the minor's participation. Any release of claims relating to the minor is intended to apply only to the extent permitted by applicable law.

  • Photo and Media Consent

  • High Horses Center for Equine-Assisted Services ("High Horses") occasionally takes photographs and video recordings during visits, events, and activities for use in organizational communications, educational materials, fundraising, advertising, social media, and other promotional purposes.

    By granting consent below, I authorize High Horses to photograph or record the individual identified in this form and to use their image, likeness, and, when appropriate, first name in these materials without compensation.

    I understand that photographs and recordings may be shared publicly, including online, and that I may withdraw my consent for future use by contacting High Horses. Withdrawal of consent will not affect materials already published or distributed.

    Photo and media consent is voluntary and is not required to visit or participate in authorized activities at High Horses.

    I understand that my signature indicates my agreement to the applicable terms of this release. If I am signing on behalf of a minor, I confirm that I am the minor's parent or legal guardian, that I am authorized to provide consent, and that I consent to the minor's visit and participation in activities authorized by High Horses.

    I understand that my selection regarding Photo and Media Consent is separate from my agreement to the liability release.
