Hello, welcome to my booking form!

In order to help me streamline the booking process please fill out any relevent information I may need for your appointment below.
Please note the following:

The styles I am known for are: Blackwork, Floral, Illustrative, Ornamental and Small Colour.

I do not tattoo the following styles: Realism, Large Colour Pieces.

I do not currently tattoo the following areas: Necks, Faces

LOCATION:

I am situated at Maddog Tattoo Studio, Unit 3, Bolebridge Mews, Tamworth, Staffordshire, B79 7PA.
We have often been confused with the Australian Tamworth so please be aware before booking that we are located in the UK.

Please note, any script in another language, I am unable to redesign or translate seeing as I can only read and write in English. Please make sure if it’s in another language the spelling and script is correct.

I do not directly copy from reference images provided, I take inspiration and create a new design suited to my client’s requests. Please click ‘I agree’ if you understand these terms.
BOOKING INFORMATION

*Valid photo ID is required for ALL appointments (This can either be a passport or driving license)*

* As our studio is quite small, we politely request that you come alone to your appointment unless you've already asked in advance.

* A deposit must be made to secure your appointment, failure to do so within 24 hours of the arrangement may risk losing the chosen date/timeslot.

* All deposits are NON REFUNDABLE, however can be transferred if more than 48 hours notice is given where applicable.

* If an appointment is rescheduled, a new date and timeslot will have to be decided within a week of cancelling the original appointment, otherwise the deposit will be lost.

* You may be able to reschedule your appointment ONCE if notified before 48 hours of the existing appointment. Any further changes will require a fee.

* There is a 15 minute grace period for all appointments, failing to attend within this will result in a deposit loss.
