As the person completing this form I agree and understand that I am responsible to pay for all classes the above student was enrolled in up until the end of the required 4 week notice period as per the Terpsichore Dance Company enrolment terms and conditions agreed upon at the time of registration.

I understand there are no refunds for tuition fees as per the Terpsichore Dance Company enrolment terms and conditions agreed upon at the time of registration.

I understand that the above dancer cannot attend classes after the final class date (listed in the withdrawal confirmation email) and that this includes any make-up classes owed. (This can be disregarded if still attending other classes for the season)

I understand that membership fees (selected at the time of registration) must be paid in full prior to withdrawing fully from all classes.

All miscellaneous charges are due in full before the end of the 4 week notice period. I understand that failure to pay any outstanding amounts will result in Terpsichore Dance Company sending my account to a collection agency where I will also be responsible to pay collection fees in addition to any outstanding fees. 

I understand I will be charged a $25 administration fee upon processing this withdrawal. (Note: if you are continuing with other classes at TDC this admin fee will be waived)
