Statement of Consent & Waiver

We would like to enroll our child/Children (names mentioned above) for Fauji Ki Paathshala Boot Camp from (dates selected above) Our child/Children is/are fit to participate in the camp We understand the camp design and our child is medically fit to participate in this camp. Our child does not suffer from any known condition - specifically heart related, blood pressure related, acute asthma, epilepsy, or serious allergies and reactions - that could get aggravated due to the experience or would render the camp unsafe for him/her or others.

Our consent here also includes permission to Fauji ki Paathshala/ their associates and the doctor or hospital chosen by them to provide medication or hospitalization in case of need. We understand the communication protocol during the course of the camp.

We understand that we will not be able to communicate directly with my child during the camp. Fauji ki Paathshala/ their associates will provide updates through their social media channel. We understand that this is not a commitment, and is dependent on telephone and internet connectivity at the camp site. We also understand that the updated photographs will be representative and generic and may or may not cover our child specifically. We are fine with Fauji ki Paathshala/ their associates uploading our child’s photographs and videos on its social media. Fauji ki Paathshala/ their associates may use the images and videos featuring our child for its brochures, website and promotional material.

We understand the programme details and inherent risks of outdoor camps:While Fauji ki Paathshala/ their associates has trained and experienced staff and does its best, we understand and acknowledge the risks associated with outdoor programmes and the related logistics of travel and stay. These are same as risks present in any private or family trip in the same setting or with similar activities. We release Fauji ki Paathshala/ their associates, their employees, instructors, doctors, contractors and directors of any liability from claims arising due to our child / children participation in the camp and the related travel and activities conducted therein.
