Description: Youth will visit Ripleys Aquarium for team bonding and to appreciate the beauty of God's Creation and fellowship over food.

Transportation:

Members are responsible for their own transportation.

Cost: $20 (Cash ONLY to Sis.Erica)

Other:

Members must be dressed appropriately for the weather

This section is intended to obtain consent for the use of images, video, and audio recordings taken by designated members of North Victory Baptist Church. By signing this consent form, you acknowledge and agree to the terms outlined below.

1. Consent to Record and Use Media I, the parent/guardian, hereby give my consent to North Victory Baptist Church and its representatives, affiliates, and partners to record, photograph, and/or video my child during the aforementioned event or activity.

2. Usage of Media I, understand that the images, video, and/or audio recorded may be used for the following purposes:- Promotional material, advertisements, and marketing purposes (including social media, websites, printed publications, etc.), Educational, training, and documentary purposes.

3. No Obligation to Participate My Child is under no obligation to participate in any media recording or to provide consent. I also understand that I can withdraw my consent at any time prior to the release or distribution of the media. To withdraw consent, I will notify North Victory Baptist Church.

By signing below, I acknowledge that I have read and understood the terms of this consent form, and I will provide my consent or dissent below.
I give permission to my child to attend this Youth Ministry Activity under North Victory Baptist Church. I understand it is my child's responsibility for transportation there and back from the venue. I hereby waive, release, and discharge any and all claims for damages for personal injury, property damage or which may hereafter occur to my child as a result of their participation. It is my child's responsibility to bring any medication regarding any allergies as a precaution and that they understand what food they can or cannot eat. It is also advised for your child to physically have their health care if medical care is needed. This release is intended to discharge in advance North Victory Baptist Church, its leadership, officers, volunteers, and representatives from liability, even though that liability may arise out of perceived negligence on the part of persons mentioned above. It is further understood and agreed that this waiver, release, and assumption of risk is to be binding on my heirs and assignees.
