  • I give the staff of Minnalousha May's Cat Hideaway permission to verify any information provided on this application. I understand that completion of this application DOES NOT guarantee placement of an animal in my home. I certify that the above information is true and correct.

  • I hereby authorize MMCH to use a photograph of me with my adopted animal in any and all publications, including but not limited to online social media. I acknowledge that since my participation is voluntary, I will receive no financial compensation for use of the photograph.
