I understand and acknowledge the following:

Photo and Video Release: I grant permission for photographs and videos of my child(ren) and myslef/family to be taken during the camp for promotional, educational, and archival purposes by Duncraig Christian Community Church. I understand that these images may be used in printed materials, online media, and presentations related to the Church Camp.

Payment: I authorize Duncraig Christian Fellowship (DCCC) to deduct the above funds from my debit/credit card as a one off single payment.

Liability I release DCF Inc. T/As Duncraig Christian Community Church, its staff, volunteers, and affiliates from any liability for injuries, accidents, or loss of personal property that may occur during the Church Camp.
