  • Eligibility Snapshot
    Applicants must typically be:

    ✅ 18+
    ✅ Able to live independently in a shared setting
    ✅ Have verifiable income OR a confirmed placement plan
    ✅ Willing to follow house rules and accountability standards

  • When the Program Is Not a Fit
    A referral may not be approved if the individual:

    Has no verifiable income or confirmed plan
    Requires 24/7 supervision or clinical-level care
    Is unable to safely live in a shared environment
    Has unresolved safety concerns that cannot be supported in our model

    Option A: Standard Placement Coordination
    $150 – $250 (One-Time Fee)
    Best for referrals with documentation ready and flexible move-in timeline.

    Includes:

    Referral review
    Intake screening + verification
    Placement match (based on availability)
    Move-in coordination

    Option B: Priority Placement Coordination
    $300 – $450 (One-Time Fee)
    Best for referrals requiring faster turnaround and additional coordination.

    Includes everything in Option A, plus:

    expedited scheduling
    same-week placement attempts
    enhanced verification follow-up

    Option C: Emergency / Rapid Transition Placement
    $500 – $750 (One-Time Fee)
    Best for urgent discharge, shelter exit, or time-sensitive placement needs.

    Includes everything above, plus:

    urgent placement review
    move-in readiness coordination
    immediate onboarding support
    This option is best for 24–72 hour discharge situations


    Ongoing Partner Support (Optional Add-On)
    $100 – $200/month per referral (Optional)
    For organizations wanting structured follow-up confirmation.

    May include:

    monthly stability confirmation (non-clinical)
    attendance to check-ins verification
    basic progress notes for housing compliance
    placement status updates

  • Billing & Payment Terms
    Placement Coordination fees are billed to the organization, not the resident.

    We provide:

    Invoice with service description
    Written confirmation of placement status
    Clear documentation of what was completed
