Liability Waiver & Informed Consent
Mayfield Sportsplex - Mayfield Sports Camps - Summer 2026

1. Acknowledgment of Risk
I, the undersigned, understand that participation in Mayfield Sports Camps involves physical activity which may include, but is not limited to, soccer, basketball, baseball, and other multisport activities. I acknowledge that these activities carry inherent risks, including:

Injuries arising from physical exertion or contact with other participants.
Injuries resulting from the use of sports equipment or facility surfaces.
Unforeseen incidents occurring while the child is under the supervision of camp staff.

2. Waiver and Release of Liability
In consideration of my child being permitted to participate in the camp, I hereby release and hold harmless Mayfield Sports Camps, its owners, directors, employees, and volunteers from any and all liability, claims, or causes of action arising out of any loss, damage, or injury, including death, that may be sustained by my child, except in cases of gross negligence by the camp.

3. Medical Authorization & Sun Safety
As the child’s parents/guardians are not present during camp hours:
I authorize Mayfield Sports Camps staff to seek emergency medical treatment for my child if necessary.
I confirm my child is physically fit to participate.
I give permission for staff to assist in the application of sunscreen if required.

4. Code of Conduct & Dismissal
I understand that for the safety of all children, Mayfield Sports Camps reserves the right to dismiss any child whose behavior is disruptive or dangerous to others, without a refund of camp fees.

5. Governing Law
This agreement shall be governed by and construed in accordance with the laws of the Province of Ontario and the federal laws of Canada applicable therein.

Refunds & Credits Policy
Please Note: All registrations for Mayfield Sports Camps are final. No cash, check, or credit card refunds will be issued for any reason, including but not limited to: participant illness, personal scheduling conflicts, or inclement weather. At the sole discretion of Mayfield Sports Camps management, a program credit may be issued for future camp sessions or programs. Credits are non-transferable and must be used within [12 months] of the date of issuance.
