Combined Consent & Release Agreement
By submitting this form, I acknowledge that participation in the Glowing Over Bullying event on March 7, 2026, hosted by Kylah McNeil, is voluntary. I understand that normal movement and physical activity may occur during the event. I release and hold harmless the host, organizers, volunteers, sponsors, and partners from liability for accidental injury, illness, or property damage that may occur during participation.
In the case of a medical emergency, I authorize appropriate emergency medical care to be administered and accept financial responsibility for any related expenses. Efforts will be made to contact the listed emergency contact prior to treatment when reasonably possible.
I understand that photos and videos may be taken during the event for documentation, awareness, youth advocacy, and promotional purposes including social media, websites, printed materials, and press. By granting media consent, I authorize the use of my/my child’s image without compensation and understand that once posted publicly, images may be shared beyond the organizer's control.
