We are thrilled to have your family join MARKED this year!

This form will provide information on your family and then you will complete a separate form to register your camper(s).

After this Family Registration, you will hit the SUBMIT button and then go to the Camper Registration.

Campers must be signed in and out each day by a responsible adult. Please provide the names and phone numbers (including area codes) of all adults—parents included—who are authorized to drop off and/or pick up your child. Only the adults listed will be permitted to pick up your child, unless you have completed an authorization form allowing a camper age 16 or older to sign themselves in and out. If this is your intent, please request the form from the Camp Director, Joella VanOosterhout, and note below that your child will be completing it.*

PARENT: I hereby release, absolve, indemnify and hold harmless Lakeshore Christian Youth Arts, Living Word Church, and its staff, camp leaders and volunteers in case of injury during normal routines and activities pertaining to camp activities and performances. I hereby waive all claims against the above. I assume all risks incidental to the conduct of these activities. I assume transportation to and from camp daily. I also give permission for the Lakeshore Christian Youth Arts and Living Word Church to use any pictures or videos of my child in photographs and videography related to the MARKED Youth Arts Camp on the website, social media, or displays for informational/communication and promotional purposes only. PARENT: I understand this is a day camp run by volunteers with the goal of encouraging students to grow in their Christian faith and artistic development so in their lifetimes they can serve the Church and the World. PARENT: I understand that, depending on their workshops, my student will need to bring any camp materials (computer, musical instrument, camera, etc.) each day. (This will be communicated via email prior to camp week.)They will also need to wear their designated MARKED 2026 camp shirt and either blue or black pants/long shorts for the Friday evening Showcase program. PARENT: I understand that my child(ren) can be dropped off between 8:45 and 9:00 a.m. and they are to be picked up between 3:00 and 3:15 p.m. (Monday through Thursday). I understand that if they are early (before 8:45 a.m. Tuesday through Friday) or late (after 3:15 p.m. Monday through Thursday) my camp account will be assessed a $10 fee for each event, unless there is a family emergency communicated to the camp director. During the lunch time, campers may bring their own lunch or pre-order a camp lunch. Water and lemonade will be provided during lunch and breaks. Please note that refrigeration will not be available, so campers who bring their own lunches should plan accordingly. Camp lunches must be pre-ordered for the full week and will be simple meals (such as hot dogs, sloppy joes, or chicken nuggets) with fruits and veggies. Special dietary needs cannot be accommodated. If you choose a camp lunch, please select this option during registration for each camper and pay the lunch fee at that time. PARENT: I agree that I will either provide a lunch for my student or prepay $30 for camp lunches, and I understand that special dietary needs cannot be accommodated.*

PARENT: I understand that: my family is invited to a picnic meal Friday at 5:30 p.m., the final program ("Showcase") is Friday at 6:30 p.m., and parents and grandparents are invited.*
