Liability Waiver, Assumption of Risk & Release Agreement

Please read each section carefully. By signing this form, you acknowledge your full understanding and agreement.

1. Voluntary Participation

I understand that I am voluntarily participating in services offered by Inner Temple Wellness, including but not limited to:

Energetic session
Yoga classes or Stretch classes
Meditation or breathwork sessions
Wellness workshops
I understand these services are not a substitute for medical care, diagnosis, or treatment.

2. Assumption of Risk

I acknowledge that participation in yoga, meditation, and wellness sessions may involve physical movement, breathwork, and emotional release. I understand that:

Physical activity may present risks of injury.
Energy work may create emotional or physical sensations.
I am responsible for listening to my body and deciding what is safe for me.
I agree to take full responsibility for my health and wellbeing during sessions.

3. Health Disclosure

I confirm that:

I have disclosed any relevant physical or mental health conditions to my instructor or practitioner.
I will update Inner Temple Wellness if my health status changes.
I will consult my doctor before participating if I have any concerns.
 

4. No Medical Advice, No Guarantees & Alternative Therapy Disclosure

I understand and acknowledge that:

Practitioners at Inner Temple Wellness are not medical professionals and do not diagnose, treat, or prescribe.
All services, including energy work and yoga, are considered complementary or alternative wellness practices.
No statements, services, or recommendations made by Inner Temple Wellness are intended to replace medical or psychological treatment.
5. Release of Liability & Indemnification

In consideration of participating in services, I hereby:

Release and forever discharge Inner Temple Wellness, its owners, instructors, employees, independent contractors, and affiliates from any and all liability, claims, demands, causes of action, or damages of any kind, including those arising from negligence, whether known or unknown.
Waive any right to sue or bring legal action for personal injury, emotional distress, property damage, or any other loss that may occur before, during, or after participation.
Agree to indemnify and hold harmless Inner Temple Wellness and its representatives from any claims brought by me, my family, estate, or legal representatives.
This release applies to all current and future sessions and remains in effect indefinitely unless revoked in writing.

6. Personal Belongings

I understand that Inner Temple Wellness is not responsible for lost, damaged, or stolen items.

7. Photography & Media Consent (Optional)

I give permission for Inner Temple Wellness to use photos or videos of me taken during sessions for marketing or social media purposes.

8. Communicable Illness Acknowledgment

I understand that participation may include exposure to communicable illnesses. I assume full responsibility for any such exposure and its potential consequences.

I further release, indemnify, and hold harmless Inner Temple Wellness from any claims related to illness, infection, or transmission of communicable diseases.

**## 9. Arbitration, Governing Law & Severability

I agree that:

Any dispute arising from participation or this agreement will be resolved exclusively through binding arbitration in the State of Arizona.
This agreement shall be governed by Arizona law.
If any portion of this agreement is found unenforceable, all remaining sections shall remain in full effect.

I agree voluntarily and without coercion.
I am at least 18 years old (or a parent/guardian is signing for a minor).
