• Welcome! This form includes important information regarding our HIPAA Notice of Privacy Practices, Practice Policies, Informed Consent, and Insurance & Financial Responsibility Statement. Please read through each section. We are here to answer any questions you may have regarding our policies.
  • Northwest Pediatric Therapy is a therapy partnership owned and operated by two individual therapy practices, Northwest Pediatric Speech Therapy, owned and operated by Megan McMaster-Stine, and Sprout Pediatric Therapy, owned and operated by Lindsey Casper (herein referred to as Northwest Pediatric Therapy or Northwest Pediatric Therapy and associates). Collectively, we strive to work together to build strong relationships with the families we serve to receive the best clinical outcomes. The following practice policies help us maintain this standard of care.

  • HIPAA NOTICE OF PRIVACY PRACTICES

THIS NOTICE DESCRIBES HOW HEALTH INFORMATION MAY BE USED AND  DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION.  PLEASE REVIEW IT CAREFULLY.

    I. OUR PLEDGE REGARDING HEALTH INFORMATION:

We understand that health information about you and your health care is  personal. We are committed to protecting health information about you. We create a record of the care and services you receive from us. We need this record to provide you with quality care and to comply with certain legal requirements. This notice applies to all of the records of your care generated by this practice. This notice will tell you about the ways in which we may use and disclose health information about you. We also describe your rights to the health information we keep about you, and describe certain obligations we have regarding the use and disclosure of  your health information. We are required by law to:

Make sure that protected health information (“PHI”) that identifies you is kept private.

Give you this notice of our legal duties and privacy practices with respect to health information.

Follow the terms of the notice that is currently in effect.

We can change the terms of this notice, and such changes will apply to all  information we have about you. The new notice will be available upon request, in our office, and on our website.

II. HOW WE MAY USE AND DISCLOSE HEALTH INFORMATION ABOUT YOU:

The following categories describe different ways that we use and disclose health information. For each category of uses or disclosures we will explain what we mean and try to give some examples. Not every use or disclosure in a category will be listed. However, all of the ways we are permitted to use and disclose information will fall within one of the categories.

Most importantly, your information may be shared with other healthcare  providers within the same office to provide optimal care. This includes any  member of the Northwest Pediatric Therapy group: Northwest Pediatric Speech Therapy LLC and Sprout Pediatric Therapy. Your information is used to provide referrals, exchange of information for interdisciplinary collaboration, and, but not limited to, coordination of care at our office facility.

For Treatment Payment, or Health Care Operations: Federal privacy rules  (regulations) allow health care providers who have direct treatment relationship with the patient/client to use or disclose the patient/client’s personal health information without the patient’s written authorization, to carry out the health care provider’s own treatment, payment or health care operations. We may also disclose your protected health information for the treatment activities of any health care provider. This too can be done without your written authorization. For example, if a health care provider were to consult with another licensed health care provider about your condition, we would be permitted to use and disclose your personal  health information, which is otherwise confidential, in order to assist the health care provider in diagnosis and treatment of your condition.

Disclosures for treatment purposes are not limited to the minimum necessary standard. Because other health care providers need access to the full record and/or full and complete information in order to provide  quality care. The word “treatment” includes, among other things, the coordination and management of health care providers with a third party, consultations between health care providers and referrals of a patient for health care from one health care provider to another.

Lawsuits and Disputes: If you are involved in a lawsuit, we may disclose health information in response to a court or administrative order. We may also disclose health information about your child in response to a subpoena, discovery request, or other lawful process by someone else  involved in the dispute, but only if reports have been made to tell you  about the request or to obtain an order protecting the information requested.

III. CERTAIN USES AND DISCLOSURES REQUIRE YOUR AUTHORIZATION:

    1. Session Notes: We do keep “Session notes” and any use or disclosure of such notes requires your authorization unless the use or disclosure is:

    a. For our use in treating you.

    b. For our use in training or supervising associates to help them improve their clinical skills.

    c. For our use in defending the practice in legal proceedings instituted by you.

    d. For use by the Secretary of Health and Human Services to investigate our compliance with HIPAA.

    e. Required by law and the use or disclosure is limited to the requirements  of such law.

    f. Required by law for certain health oversight activities pertaining to the  originator of the session notes.

    g. Required by a coroner who is performing duties authorized by law.

    h. Required to help avert a serious threat to the health and safety of  others.

    2. Marketing Purposes.

As a healthcare provider, we will not use or disclose your PHI for marketing purposes.

    3. Sale of PHI.

As a healthcare provider, we will not sell your PHI in the regular course of our business.

IV. CERTAIN USES AND DISCLOSURES DO NOT REQUIRE YOUR AUTHORIZATION.

Subject to certain limitations in the law, we can use and disclose your PHI  without your Authorization for the following reasons:

    1. When disclosure is required by state or federal law, and the use or disclosure complies with and is limited to the relevant requirements of such law.

    2. For public health activities, including reporting suspected child, elder, or  dependent adult abuse, or preventing or reducing a serious threat to anyone’s health or safety.

    3. For health oversight activities, including audits and investigations.

    4. For judicial and administrative proceedings, including responding to a court or administrative order, although our preference is to obtain an authorization from you before doing so.

    5. For law enforcement purposes, including reporting crimes occurring on our premises.

    6. To coroners or medical examiners, when such individuals are performing  duties authorized by law.

    7. For research purposes, including studying and comparing the patients  who received one form of care versus those who received another form of care for the same condition.

    8. Specialized government functions, including, ensuring the proper execution of military missions; protecting the President of the United  States; conducting intelligence or counterintelligence operations; or,  helping to ensure the safety of those working within or housed in  correctional institutions.

    9. For workers’ compensation purposes. Although our preference is to obtain an authorization from you, we may provide your PHI in order to comply with workers’ compensation laws.

    10. Appointment reminders and health related benefits or services.

We may use and disclose your PHI to contact you to remind you that you have an appointment with us. We may also use and disclose your PHI to tell you about treatment alternatives, or other health care services or benefits that we offer.

    V. CERTAIN USES AND DISCLOSURES REQUIRE YOU TO HAVE THE OPPORTUNITY TO OBJECT.

    1. Disclosures to family, friends, or others. We may provide your PHI to a family  member, friend, or other person that you indicate is involved in your care  or the payment for your health care, unless you object in whole or in part.  The opportunity to consent may be obtained retroactively in emergency situations.

VI. YOU HAVE THE FOLLOWING RIGHTS WITH RESPECT TO YOUR PHI:

    1. The Right to Request Limits on Uses and Disclosures of Your PHI. You have  the right to ask me not to use or disclose certain PHI for treatment, payment, or health care operations purposes. We are not required to agree to your request, and we may say “no” if we believe it would affect your health care.

    2. The Right to Request Restrictions for Out-of-Pocket Expenses Paid for In Full. You have the right to request restrictions on disclosures of your PHI to health plans for payment or health care operations purposes if the PHI pertains solely to a health care item or a health care service that you have paid for out-of-pocket in full.

    3. The Right to Choose How We Send PHI to You. You have the right to ask our office to contact you in a specific way (for example, home or cell phone) or to send mail to a different address, and we will agree to all reasonable requests.

    4. The Right to See and Get Copies of Your PHI. Other than “session notes,”  you have the right to get an electronic or paper copy of your medical record and other information that we have about you. We will provide you with a copy of your record, or a summary of it, if you agree to receive a summary, within 30 days of receiving your written request, and we may charge a reasonable, cost based fee for doing so.

    5. The Right to Get a List of the Disclosures We Have Made. You have the right to request a list of instances in which we have disclosed your PHI for purposes other than treatment, payment, or health care operations, or for which you provided our office with an authorization. We will respond to your request for an accounting of disclosures within 60 days of receiving your request. The list we will give you will include disclosures made in the last six years unless you request a shorter time. We will provide the list to you at no charge, but if you make more than one request in the same year, we will charge you a reasonable cost-based fee for each additional request.

    6. The Right to Correct or Update Your PHI. If you believe that there is a mistake in your PHI, or that a piece of important information is missing from your PHI, you have the right to request that we correct the existing information or add the missing information. We may say “no” to your request, but we will tell you why in writing within 60 days of receiving your request.

    7. The Right to Get a Paper or Electronic Copy of this Notice. You have the right to get a paper copy of this notice, and you have the right to get a copy of this notice by email. And, even if you have agreed to receive this notice via email, you also have the right to request a paper copy of it.

  • INFORMED CONSENT FOR THERAPY SERVICES

I hereby request and consent to Northwest Pediatric Therapy to perform treatment and care for my child as prescribed by a physician and/or recommended by his or her occupational therapist and/or speech language pathologist.

    1. I understand and am informed that, as in the practice of medicine, occupational and/or speech-language therapy may have some risks. I understand that I have the right to ask about these risks and have any questions answered about my child's condition prior to treatment.

    2. I acknowledge and agree that my child will be evaluated and treatment completed at their developmental disability agency, early learning center, in the home, or at Northwest Pediatric Therapy clinic, with or without the presence of parent or guardian.

    3. I consent and authorize Northwest Pediatric Therapy to administer treatment under the direction and supervision of a licensed occupational therapist or licensed speech-language pathologist, or graduate practicum or intern student who is being directly supervised by a licensed occupational therapist or licensed speech-language pathologist.

    4. I understand that all service payments are due at the time of service, and that some therapy may not be covered by insurance. Northwest Pediatric Therapy will alert me as soon as possible about any portion of payment that is not covered, and I understand that payment is due immediately upon receipt of that information.

    5. I understand that I am responsible for knowing current insurance coverage for the patient receiving services. It is my responsibility to report to Northwest Pediatric Therapy all coverage, including multiple insurers, so that Northwest Pediatric Therapy can properly bill for services. If I do not properly report insurers, I may be responsible for out-of-pocket expenses.

    6. I have carefully read and fully understand this Informed Consent Form and have had the opportunity to discuss it with a member of Northwest Pediatric Therapy if I choose. I agree to hold Northwest Pediatric Therapy harmless for claims or damages in connection with treatment. This is a contract between myself and Northwest Pediatric Therapy, and | understand that it is also a release of potential liability.

  • PRACTICE POLICIES

    TELEPHONE AND COMMUNICATION ACCESSIBILITY

We are often seeing clients throughout the day. We try to make ourselves as available to you as possible, but we also take on the roles, not only of clinicians, but as billers for your insurance, schedulers for your appointments, and other administrative needs for your therapy. This is largely due to the size of our clinic and to provide you with the most detailed and personalized care. If you need to contact us between sessions, please leave a message on voicemail, text, or email your provider. We are often not immediately available; however, we will attempt to return your call within 24 hours. If a true emergency arises, please call 911 or any local emergency room.

    APPOINTMENTS

The standard meeting time for treatment for speech therapy is 30 minutes depending on your provider's recommendations. The standard meeting time for treatment for occupational therapy is 45-60 minutes depending on your provider's recommendations. Evaluations can take 60-180 minutes, depending on needs. You will receive a comprehensive report detailing outcomes of evaluation measures. The report will include standardized measures that compare to others of the same age if the child can complete standardized measures. A plan of care will be developed given input from your family that will address weaknesses and will benefit your child the most.

    CANCELLATIONS

Please remember to cancel or reschedule 24 hours in advance. Patients may be removed from the schedule for the following reasons:

Three consecutive no-show / late-cancellation appointments (less than 24-hour notice). Inconsistent attendance (including, arriving late for appointments) for less than 75% of scheduled appointments over a 3-month time period. In the event of any of the above reasons, therapy patients may be removed from the schedule and placed on a waitlist. If you are removed from the schedule because of attendance problems, returning to therapy will require approval of the treating therapist.

A minimum of 24-hours notice is requested for cancellations and rescheduling requests. We do understand that illnesses and emergencies happen suddenly and may require a last-minute notification.

    ILLNESS

Attendance when sick is not encouraged for the health and safety of others. In the best interests of your child, please try to reschedule their appointment if a cancellation is required. If your child is showing symptoms, we will all use our best judgment to decide if your appointment needs to be cancelled or rescheduled. For example, if you child is exhibiting a fever and other related symptoms, it is best if their appointment is cancelled. If you child is exhibiting allergy related symptoms, they may still be able to attend their appointment. However, if these symptoms are in relation to others having been ill recently, it is more prudent to reschedule. Please call or text your therapist to discuss if an appointment should be rescheduled.

    WEATHER

We also understand that winter weather conditions can pose challenges and at times are unsafe for driving. In these situations, we recommend requesting a switch to teletherapy services or rescheduling your appointments.

    ARRIVE ON TIME

Punctuality for appointments allows adequate therapist/patient interaction and time to report outcomes to parents. If you are 10 minutes or more late, you may not be able to be seen at that time and could have to wait until your next scheduled appointment.

    TELEPRACTICE

Northwest Pediatric Therapy offers teletherapy options. Your sessions may be conducted on a secure (encrypted from end to end) online platform. Telepractice may be deemed appropriate for your family if transportation is difficult, to provide more sessions given difficulty scheduling multiple in-clinic sessions, to assist with generalization of skills, given environmental or health concerns and/or to maintain plan of care recommendations.

    MINORS

If you are a minor, your parents may be legally entitled to some information about your treatment. We will discuss with you and your parents what information is appropriate for them to receive, and which issues are more appropriately kept confidential.

    TERMINATION

Ending relationships can be difficult. Therefore, it is important to have a termination process in order to achieve closure. The appropriate length of the termination depends on the length and intensity of the treatment. We may terminate treatment after discussion with you if we determine that the treatment is not being effectively used or if you are in default on payment. We will not terminate the therapeutic relationship without first discussing and exploring the reasons and purpose of terminating. If treatment is terminated for any reason or you request another provider, we will provide you with a list of qualified professionals to treat you. You may also choose someone on your own or from another referral source. Should you fail to schedule an appointment for three consecutive weeks, unless other arrangements have been made in advance, for legal and ethical reasons, we must consider the professional relationship discontinued.

Termination may also be a result of difficult behavior in your child. We provide services in a quiet office setting. Behaviors that include aggression, oppositional-defiance, disruption of other office residences in the building, or are deemed unmanageable in any capacity by the treating physician, following consultation with any team members that are specifically skilled in behavior management, will be terminated from therapy.

  • PAYMENT POLICY AND FEE SCHEDULE

Billing can be complicated. This is especially true when insurance is involved. We want to be transparent when it comes to our billing policy and fee schedule. Our fee schedule will look different if you are using insurance based on your benefits policy or making payments out of pocket. Speech and language therapy codes are untimed, meaning that, regardless of the time spent with you, insurance will pay us the same amount. In this case, we have set a static rate for insurance. However, we are able to set hourly rates for time-based out-of-pocket payers.

Below you will see our rates for codes that we often bill to your insurer. Sometimes we may bill both an evaluation code and a treatment code at your evaluation appointment if we are providing treatment in conjunction with your evaluation.

We offer a discounted rate for individuals that pay promptly. If you are interested in this option, please discuss it with your therapist and we will provide you with the necessary paperwork to initiate your discount. This is only available for people that are paying out-of-pocket.
