  • YOUTH CAMP 2026 STUDENT REGISTRATION

  • We're so excited to have you join us for Wake Youth Camp (WYC) 2026, presented by Wake Church in partnership with Texas Ministries. This year, WYC will offer two identical camp sessions. Once you select your church's name from the list below, this form will populate with your camp session. 

    Camp Sessions:

    • Week 1: June 5 - 9
    • Week 2: June 9 - 13

    Both sessions will be held at Boxwoods at Camp Cho-Yeh in Livingston, TX and will offer the same camp experience. Camp is open to students entering 7th grade this fall through the Class of 2026.

    All payment for camp will be coordinated through your church's TXM Youth Camp point person. This person's name will autopopulate when you select your church's name from the list below and is who you should contact with any questions regarding payment. 

    BEFORE YOU BEGIN, PLEASE READ THE FOLLOWING:

    • A completed registration is required for each student attending camp. This form is to be completed by a Camper's Parent/Guardian, NOT the Camper. 
    • Campers who are 18 will need to sign an electronic waiver at the end of this registration form. 
    • If you have medical insurance, you will need to be able to attach electronic copies of the front and back of the card to this form. 
    • Please note that all items marked with an asterisk (*) are required in order for the form to be submitted.
    • Completing this form will take 15-20 minutes.

    Per the Texas Administrative Code for youth camps and the Boxwoods Health Center Guidelines, we must collect the following information for each Camper.
  • DIET & NUTRITION: 

    Camp always has options available for gluten-free, vegetarian, and lactose-intolerant participants. While the kitchen is nut-free, some prepackaged snacks available at the camp store may contain nuts. We will do our best to accommodate your listed dietary restrictions and will reach out if we have any questions or are unable to meet a need.

  • MEDICATION:

    "Medication" is any substance a person takes to maintain and/or improve their health, prescription or over-the-counter. This includes vitamins and natural remedies. It also includes items used on an as-needed basis, such as inhalers. 

  • All medication to be administered at camp must comply with the following guidelines and be locked in the nurse's office.

    1. All medication, whether prescription or over-the-counter, must be in the original container. All prescription medication must be in the Camper's name. Sharing of prescription medication is NOT allowed. Inhalers must be accompanied by a prescription label.

    2. All medication must be accompanied by the dated medication authorization form included here, digitally signed by the parent/legal guardian. 

    3. Please include instructions for over-the-counter medications in the form below.

    4. Prescription medication will be given as directed on the label.

    5. If there has been a change in dosage, please send a note to camp from the camper's doctor reflecting the change.

  • MEDICATION AUTHORIZATION FORM:

    By electronically signing below, I request that the listed medications be given to my student as directed. I agree to hold harmless Wake Church, Texas Ministries, Camp Cho-Yeh: A Boxwoods Camp, and its employees/volunteers for any consequences resulting from administration of medication. Medication authorization will be effective the duration of camp, unless revoked by the parent/legal guardian sooner.

    The following non-prescription medications may be stocked in the nurse's office and are used on an as needed basis to manage illness and injury. Please let us know if your Camper is NOT able to take these medications.

  • PARENT/GUARDIAN AUTHORIZATION FOR HEALTH CARE

  • By signing below, I certify that this health history is correct and accurately reflects the health status of the Camper to whom it pertains. The person described has permission to participate in all camp activities except as noted by me and/or an examining physician. I give permission to the physician/nurse selected by the camp to order x-rays, routine tests, and treatment related to the health of my child for both routine health care and in emergency situations. If I cannot be reached in an emergency, I give my permission to the physician/nurse to hospitalize, secure proper treatment for, and order injection, anesthesia, or surgery for this child. I understand the information on this form will be shared on a “need to know” basis with camp staff. I give permission to photocopy this form. In addition, the camp has permission to obtain a copy of my child’s health record from providers who treat my child in the event of an emergency and these providers may talk with the program’s staff about my child’s health status.

  • WAKE YOUTH CAMP WAIVER

  • RELEASE AND WAIVER OF LIABILITY:


    This Release and Waiver of Liability (the “Release”) executed by the undersigned (the “Attendee”), from the church name, in favor of Wake Church and Texas Ministries.

    The attendee hereby freely, voluntarily, and without duress executes this Release under the following terms:

    1. Release and Waiver. Attendee does hereby release and forever discharge and hold harmless Wake Church and Texas Ministries and their successors and assigns from any and all liability, claims, and demands of whatever kind of nature, either in law or in equity, which arise or may hereafter arise from Attendee’s activities at any/all Wake Church and Texas Ministries events. Attendee understands that this Release discharges Wake Church and Texas Ministries from any liability or claim that the Attendee may have against Wake Church and Texas Ministries with respect to any bodily injury, personal injury, illness, death, or property damage that may result from Attendee’s activities with Wake Church and Texas Ministries. Attendee also understands that Wake Church and Texas Ministries does not assume any responsibility for, or obligation to, provide financial assistance or other assistance, including but not limited to medical, health, or disability insurance in the event of injury or illness.

    2. Medical Treatment. Attendee does hereby release and forever discharge Wake Church and Texas Ministries from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with the Attendee’s activities with Wake Church and Texas Ministries.

    3. Assumption of the Risk. The Attendee understands that the activities may include recreation that normally is not considered hazardous to the Attendee, but also realizes any activity may involve possible risks. The Attendee hereby expressly and specifically assumes the risk of injury or harm in these activities and releases Wake Church and Texas Ministries from all liability for injury, illness, death or property damage resulting from the activities of the Attendee’s participation with Wake Church and Texas Ministries.

    4. Insurance. The Attendee understands that, except as otherwise agreed to by Wake Church and Texas Ministries in writing, Wake Church and Texas Ministries are not obligated to and do not carry or maintain health, medical, accident, car, or disability insurance coverage for any Attendee. Each Attendee is expected and encouraged to arrive with medical or health insurance coverage in effect.

    5. Photographic Release. Attendee does hereby grant and convey unto Wake Church and Texas Ministries all right, title and interest in any and all photographic images and video or audio recordings made by Wake Church and Texas Ministries during the Attendee’s activities with Wake Church and Texas Ministries including any photographs or recordings. Due to the large number of students at said activities, exemption from these photos/videos is not possible.

    6. Other. Attendee expressly agrees that this Release is intended to be as broad and inclusive as permitted by the laws of the State of Texas, and that this Release shall be governed by and interpreted in accordance with the laws of Texas. Attendee agrees that in the event that any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release which shall continue to be enforceable.

    IN WITNESS WHEREOF, Attendee has executed this Release as of the day and year first above written.

    Electronic Signature Authorization
    By applying my electronic signature to this agreement, I agree that my electronic signature is the legally binding equivalent of my handwritten signature on paper. I will not, at any future time, claim that my electronic signature is not legally binding or enforceable.


    Parent/Guardian Authorization
    A parent or legal guardian must sign this agreement on behalf of any minor participant under the age of 18. By electronically signing this agreement, I represent that I have the authority to sign and enter into this agreement on behalf of the minor(s) listed on this form.
