Informed Consent and Acknowledgement

I hereby authorize the Director of the Ironmen Basketball to act for me, according to their best judgment, in any emergency requiring medical attention and hereby waive and release Orangewood Academy, Ironmen Basketball, Garrett James, SCC SDA, and all of their directors & coaches from any and all liability from any injuries or illness incurred while at Ironmen Basketball Camp. I will be responsible for any medical or other charges in connection with my child’s attendance. I know of no mental or physical problem which may affect my child’s ability to safely participate in this program. The undersigned agrees to indemnify and hold harmless Ironmen Athletics LLC dba: Ironmen Basketball, Garrett James, Orangewood Academy, employees, and coaches/instructors from and against alleged liability, claims, suits, damages, losses, and expenses, including attorneys’ fees arising from your child’s participation, or from any cause what-so-ever. This is not an Orangewood Academy sponsored camp; it is owned and operated by Ironmen Athletics LLC.
Refund Policy

BY ACKNOWLEDGING AND SIGNING BELOW, I AGREE TO IRONMEN BASKETBALL'S REFUND. I AGREE THAT THERE ARE NO REFUNDS AFTER JULY 6TH. IF I CHOOSE TO PROCESS A REFUND BEFORE JULY 6TH, A 25% REFUND TRANSACTION FEE WILL BE CHARGED TO ME.
