Please complete this form to register your child for a trial class scheduled on Fridays from 1:30–2:30 PM.

Kindly note the following guidelines:
- Only one (1) trial class is allowed per child
- One (1) adult guardian may sit in during the session
- Strictly one-adult policy will be observed during trial classes
- No make-up classes or rescheduling will be accommodated
