Under the Data Protection Act (DPA) 2018 and the General Data Protection Regulation (GDPR), you are entitled to obtain from Cora Health, confirmation as to whether we are processing personal data concerning you, as well as to request details about the purposes, categories and disclosure of such data. Healthshare has 30 days to meet this requirement once you submit your PHOTO ID. You can use this form to request information about, and access to any personal data we hold about you.
If a medical professional/consultant needs access to images to provide ongoing care they can request by email to GDPLIEP@nhs.net or call 0800 652 4157. For patients requesting their data in person at point of exam, your identity will be verified by the receptionist/admin team who MUST complete Personal Details and Verification sections below.
Please ensure you upload to Cora Health a copy of either your passport or driving licence. Please ensure your email address is correct as if it is wrong it will delay the process of getting your SAR to you.
If you have a btinternet.com or plus.com email address please note that we are being blocked by your service provider. We have tried to contact the service provider without success.
You can unblock as follows:
Log on to your email account.
Click on your username and then click Settings.
Choose Mail > Blocked Senders.
