Applicants should use the following questions to structure their nomination.
1. What major Lean or continuous improvement initiative(s) were delivered?
Describe the key project or programme, its scope, objectives, and the problem(s) it set out to address. Explain why this work was significant for the organisation or system.
2. What measurable impact was achieved as a result of this work?
Provide clear before-and-after measures demonstrating impact. This may include improvements in performance, quality, cost, delivery, safety, customer or user experience, or staff engagement.
3. How was the work planned, governed, and delivered?
Explain the approach taken to design, manage, and deliver the improvement. Include details of governance, stakeholder involvement, use of improvement methods, and how risks and progress were managed.
4. How have the results been sustained over time?
Describe how improvements have been embedded into standard ways of working. Include evidence of controls, capability building, leadership behaviours, or routines that ensure benefits are maintained.
5. How has learning from this work been transferred or scaled?
Demonstrate how knowledge, capability, or approaches have been shared beyond the original project. Include examples of replication, adaptation, or wider organisational impact.
