Quilt Entry Form

Please submit a quilt entry form for each quilt you are entering into the show. You do not have to be a member of Evergreen Quilters Guild to enter a quilt in the show.

If you do not want to enter your quilt online, you can use the Printable Entry Form and mail it in with your payment. Also, please refer to the Entry Information for important dates and details about your quilt show entry and the show itself!

Cost for members is $8.00. Cost for non-members is $10.00. After submitting your entry below, you will have the option to submit additional entries. When you have completed submitting your quilt entry (or entries), the Thank You page will have instructions for mailing your entry fee(s).

Completed forms must be submitted on or before March 14, 2026.
The Evergreen Quilt Show Committee classifies entries and reserves the right to reject any quilts that are soiled, damaged, inappropriate, or exceed available display space. Tied quilts or cheater quilts are not acceptable except in Youth or Non-judged categories. By submitting this form, exhibitor holds harmless and waives all legal rights in connection with any loss or damage to any display item submitted to the Evergreen Quilters Guild. Submission of this entry form implies acceptance of these terms. Entrants are responsible for their own insurance.
