Social Media Release Form

This form is to give permission for me (Releasee)  to use any digital content taken during your appointment to be used for social medias/marketing purposes for this business. Models (Releasor) will receive a discounted rate for service in exchange for photo/video content to be taken, there will be no extra monetary compensation for this exchange. This form also grants the releasee to creatively alter or edit media taken to tailor to its marketing purpose.

This content may be photo, or video recording for marketing / social media posts, stories, or video reels (Instagram, Facebook, Tiktok). Upon signing this form you grant permission for the content to be published, this form is only needed once remain valid. This form also acknowledges release of liability upon sharing the media taken. 

RELEASE

I, (Releasor) , grants permission to Hair By Alise RayAnn (Releasee),  to use my image for media (photograph image and/or video footage and/or audio recordings). 

I likewise understand that I may be identifiable from the reproduction of the media by the Releasee indicated herein this agreement.

I hereby waive my rights to inspect or approve the media for production that may be used by the Releasee for purposes stated herein, from hereon forward.

I likewise waive any right to royalties or any compensation arising from or related to the use of the image. 

The Releasor grants the Releasee creative permission to alter the photographs, provided that the photographs are not altered in an explicit manner or used to
maliciously represent the Releasor or their associates.

In giving this consent, the Releasor releases the Releasee from liability for any violation of any personal or proprietary right the Releasor may have in connection
with all third parties' use of the images on social media.

The Releasor certifies that they are 18 years of age or older. If the Releasor is under
the age of 18, their parent or guardian must sign this Release.
