NOTE: A PERSON ACCEPTS THAT BY COMPLETING AND SUBMITTING THIS FORM THAT THEY ARE REGISTERED AS A MEMBER OF THE PRETORIA HARLEQUIN HOCKEY CLUB FOR THE 2026 OUTDOOR SEASON AND ARE LIABLE FOR THE APPLICABLE FEES.

1. BY COMPLETING AND SUBMITTING THIS ONLINE FORM, I AGREE AND ACCEPT THAT I AM:

1.1.FORMALLY REGISTERED AS AN OFFICIAL MEMBER OF PRETORIA HARLEQUIN HOCKEY CLUB (“the CLUB”) FOR 2026 AND ARE BOUND BY THE CLUB’S TERMS AND CONDITIONS AS SET OUT HEREIN AND AS POSTED ON THE CLUB’S WEBSITE.

1.2. LIABLE FOR ALL CLUB AFFILIATION FEES FROM THE DATE OF REGISTRATION.

1.3. LIABLE TO PAY ALL CLUB AFFILIATION FEES IN ONE INSTALMENT BY NO LATER THAN 30 MARCH 2026 UNLESS I ENTER INTO A WRITTEN PAYMENT PLAN WITH THE CLUB.

2. BY COMPLETING AND SUBMITTING THIS ONLINE FORM, I AGREE, ACCEPT AND UNDERSTAND THAT:

2.1.The information provided by this online form may be utilised by the CLUB for the registration of teams in the Northerns Senior Outdoor League and with the South Africa Hockey Association (“SAHA”).

2.2. I cannot cancel my Membership with the CLUB unilaterally.

2.3. My Membership with the CLUB can only be cancelled at the sole discretion of the CLUB’S Executive Committee.

2.4. Should I wish to cancel my Membership for any reason whatsoever, I:

2.4.1. Am required to send a formal request for cancellation in writing to harlequinhockey@gmail.com, which request should be accompanied by my brief reason(s) for wanting to cancel my Membership.

2.4.2. Understand that NO WhatsApp communication for cancellation of my Membership will be accepted.

2.4.3. The CLUB’s Executive Committee shall consider my request for cancellation and the outcome of their decision will be communicated to me in writing. I will in the interim remain liable for the full 2026 Membership fees, notwithstanding the date on which I cancel my Membership to the CLUB.

2.5. A discount on my Membership fees may be awarded at the sole discretion of the CLUB’s Executive Committee and I will remain liable for the full 2026 Membership fees unless advised otherwise in writing and by email by the CLUB’s Executive Committee.

2.6. The CLUB’s MEMBERSHIP FEES for 2026 exclude the SAHA fees, and members must pay these fees directly to SA Hockey via the SAHOCKEY App or website (www.sahockey.co.za). Failure to pay your SAHA Fee before League starts will deem the member ineligible to participate in Provincal League but CLUB membership will remain valid.

2.7. The CLUB’s MEMBERSHIP FEES for 2026 Outdoor Season amounts amount to:

NOTE: Northerns League Fees are yet to be confirmed by the Northerns Blues Hockey Association, therefore the Club reserves the right to revise the below Club Fees after publication of the Northerns League Fees. All member wil be given a 7 calendar day period to cancel their membership without penelaties should the revised Club Fees exceed the below values.

2.7.1. Senior Membership - R4100 (excl. SAHA Fee at R485);

2.7.2. Student Membership - R4100 (excl. SAHA Fee R485); and

2.7.3. Youth Membership (18 yrs & younger) - R4100 (excl. SAHA Fee R120).

NOTE: The fee excludes CLUB macth kit and personal equipment which are mandatory.
