Registration & Payment Information

The registration fee is $350 per athlete.

A $50 sibling discount is available for each additional sibling, up to two siblings.

All registration fees must be paid in full by March 15.

A payment plan is available as follows:
- $116 due February 15
- $116 due March 1
- $118 due March 15
Failure to complete payments by the final due date may result in forfeiture of the athlete’s spot.

Waiver/Consent

Parent authorization and medical release
I, the parent or legal guardian of the above-named child, hereby give permission for my child to participate in any and all league activities sponsored by the Ducks Track & Field Program. In the event that my child becomes ill or injured while participating in league activities and I am not available, I authorize any hospital or medical clinic to provide necessary medical treatment.

I acknowledge and assume all risks and hazards incidental to participation in the program, including transportation to and from activities, and I release the Ducks Track & Field Program from any claims arising out of injury to my child.

The Ducks Track & Field Program Board of Directors has adopted a Zero Tolerance Policy. Unsportsmanlike or inappropriate conduct of any kind by a league official, coach, participant, or spectator will not be tolerated. My signature confirms my understanding of and agreement with this policy.

I further acknowledge that I have read and understand the information on this form and agree to comply with all rules, procedures, and obligations of the Ducks Track & Field Program.
