Basic Requirements:
 

The short-term mission worker needs to:

 

1.     Maintain an active and obvious commitment to Jesus Christ.

 

2.     Be active at COT for at least 6 months prior to the date of departure.

 

3.     Be older than 11 years of age if traveling without a parent (upon approval by trip leader)

 

4.     Receive parental approval (for candidates under 18 years of age).

 

5.     Possess the ability and desire to share Christ with others.

 

6.     Demonstrate maturity and proven dependability, which is seen by peers and church leaders in his/her area of ministry.

 

7.     Attend all trip meetings/events. Absences must be cleared with the Trip Leader.

 

8.     Please print this form out, fill it out, and turn it in when it is finished. This needs to be done before scheduled mission trip.

Commitment

Two references are required to be submitted with your application using the provided Reference Form. References may not be from individuals related to you by blood or marriage, nor may they be the trip leader. You will provide the names and contact information for your references, and our team will contact each reference directly with a Reference Form to complete and submit.

This information, as with all of the application, will be held in strict confidence.  Collect all the following

documents to be included with your completed application.

 

Individuals who are related to you by blood or marriage or are the trip leader should not be references.

 

The Family Education Rights and Privacy Act of 1974 permits students the right to inspect their files. Because of the importance of preserving the confidentiality of a reference, the Act permits an applicant to waive his right of access to the reference. By signing below, the applicant willingly waives his/her right of access to see this reference, knowing that this waiver is NOT required as a condition of admission.

Statement of Faith, Character, Belief and Ordinances
 

·         We believe that the Bible is inspired by God in its entirety and is without error in the original autographs, a complete and final written revelation from God. 

·         We believe that there is only one God, who is eternal and immutable, and manifests Himself in three distinct Persons: Father, Son, and Holy Spirit.

·         We believe that Jesus of Nazareth is the promised Messiah, born of a virgin, totally without sin, God in human flesh, the One who died on the cross for our sins, was buried, rose again from the dead on the third day, and ascended to the right hand of the Father in heaven, where He now intercedes for us who believe in Him.

·         We believe that Jesus Christ is going to return in the clouds for His church before the tribulation that is coming on the whole earth, and that He will return with His saints and all His holy angels to the earth at the end of the tribulation and set up His kingdom on earth, ruling and reigning forever and ever.

·         We believe that all humanity is lost and born with a sinful nature and can only be saved by a personal faith in the death and resurrection of our Lord Jesus Christ, and not by any human merit or performance.

·         We believe that heaven is a real place where the saved will dwell forever and that hell is a literal place of torment where unbelievers will suffer forever.

·         We believe in the present ministry of the Holy Spirit, by whose indwelling the Christian is empowered to live a Godly life.  We further believe His power and gifts are available to believers today and operational in the church

·         We believe worship of God should be inspirational. Therefore, we give a great place to music in our service.

·         We believe worship of God should be intelligent. Therefore, our services are designed with great emphasis upon teaching the Word of God that He might instruct us how He should be worshiped.

·         We believe worship of God should be fruitful. Therefore, we look to His love in our lives as the supreme manifestation that we have truly been worshiping Him.

·         We believe marriage has been ordained by God.  Therefore, we recognize marriage as exclusively the legal union of one man and one woman in which such union is a lifetime commitment. Also, the Bible is clear that legitimate sexual relations are exercised solely within marriage.

·         We believe that marriage is exclusively the legal union of one genetic male and one genetic female and that this union is evidenced by a marriage ceremony. We also believe that legitimate biblical sexual relations are exercised solely within marriage. Hence, sexual activities such as, but not limited to, adultery, fornication, premarital sex, incest, polygamy, homosexuality, transgenderism, bisexuality, cross-dressing, pedophilia, and bestiality are inconsistent with the teachings of the Bible and the Church. Further, lascivious behavior, the creation, viewing, and/or distribution of pornography, and efforts to alter one's physical gender or gender-related appearance are incompatible with a true biblical witness.

 

·         We believe in the creation and God as the Creator. We believe that God created man and that He created them male and female. As such, He created them different so as to complement and complete each other. God instituted monogamous marriage between male and female as the foundation of the family and the basic structure of human society. For this reason homosexuality and all other "sexual preferences" or "orientations" are unnatural, sinful, and unacceptable to God.

I have read the Statement of Faith and understand that my service to Calvary Old Town can be rescinded by the Pastoral Staff. I also understand that I will voluntarily step down at any time should anything arise in my life that would disqualify me from serving in ministry. By signing this form, I am indicating that I am in complete submission to the leadership of Calvary Old Town.
