Dear Valued Customer:

This is to inform you that the manufacturer, Wizcure Pharmaa Ltd., is recalling all lots of the following products that are within their expiration date.

Wizcure Pharmaa is recalling the product because it was not manufactured in conformance with current good manufacturing practices. It is important to note that no adverse events have been reported.

This is a voluntary recall being carried out at the RETAIL level and applies only to products manufactured by Wizcure Pharmaa. This recall has not yet been officially and publicly classified but is being conducted with the knowledge of the Food and Drug Administration.

Our records show that your facility has received at least one shipment of the affected product on the attached product list. You are required to return all products from the affected lot in your possession. To implement this recall, please complete the attached verification form and follow these instructions:

1. Examine your stock immediately to determine if you have any product from the affected lots.

2. If so, immediately discontinue use of the affected lots.

3. URGENT - The attached “Urgent Drug Recall Response Form” must be completed and returned to us by March 3, 2026. Complete and return the attached form even if you do not have the recalled product.

4. Once we receive your completed form, we will issue you a return label to send the product back to us (if you have affected inventory). Your credit will be issued after we receive the returned product. Please allow time to receive both your return label and your credit. Once received, use your return label to ship the recalled product. Do not return your recalled product prior to receiving the RMA.

IMPORTANT INFORMATION:

If you have any stock left, you must return that product to us in order to receive credit. Hilco Vision cannot issue credit for product that was purchased from another vendor. To receive credit, all product must be returned.

This form must be returned to Hilco Vision whether you have affected product or not. Practice information must be complete in order to ensure appropriate documentation of your response.
