Volunteer Waiver, Release of Liability & Media Consent
By signing below, I acknowledge that I am voluntarily registering to participate in the Slumber Pawty event at the New Leash On Life Adoption Center. I understand that I am encouraged to raise at least $1,000 by sharing my participation with friends, family, and my social networks to support this event and the shelter’s mission. I acknowledge that I may be placed in or near animal housing areas and will be around dogs and/or cats during the event. I agree to follow all staff instructions, safety rules, and animal handling guidelines at all times. I understand that participation may involve physical activity, including walking, standing, cleaning, and lifting small items, and I am choosing to participate voluntarily.

I understand that participation in the Shelter Slumber Pawty involves working in and around animals, which can be unpredictable and may pose inherent risks, including but not limited to scratches, bites, allergic reactions, slips or falls, and other injuries. I voluntarily assume all risks associated with participating in this event.

I understand that because I may handle animals, it is important to discuss tetanus vaccination with a physician. I release New Leash On Life from any responsibility related to my decision regarding tetanus vaccination and understand that this decision is made at my own risk.

I understand that for my protection, it is recommended that I wear long pants, a shirt with sleeves, and closed-toe shoes while volunteering. I agree to follow all safety rules, policies, and instructions provided by staff.

In the event of an emergency, I authorize New Leash On Life to seek medical treatment for mevif necessary. I understand that I am responsible for any medical costs incurred.

I understand that as an unpaid volunteer, I am not covered by Tennessee State Workers’ Compensation Law for any injuries sustained while participating in this event.

In consideration of being allowed to participate in this volunteer event, I waive, release, and hold harmless New Leash On Life, its officers, directors, employees, agents, and volunteers from any and all claims, demands, or causes of action for injury, illness, loss, or damage to person or property arising from or related to my participation, except in cases of gross negligence or willful misconduct.

I grant permission for New Leash On Life to photograph and/or record video of me during the Shelter Slumber Pawty and to use these images or recordings for promotional, educational, and fundraising purposes, including but not limited to social media, website, print materials, and email communications. I understand that no compensation will be provided.
