ASSUMPTION OF RISK I understand that participation in cheerleading activities including but not limited to practices, conditioning, stunts, tumbling, performances, and cheering at basketball games—involves inherent risks of injury. These risks may include, but are not limited to, sprains, strains, fractures, concussions, or other injuries.I voluntarily allow my child to participate in AV Dream Inc. Cheerleading and fully assume all risks associated with participation.
MEDICAL CONSENTIn the event of an injury or medical emergency, I authorize AV Dream Inc. Cheerleading staff to obtain emergency medical treatment for my child if I cannot be reached. I understand that I am responsible for all medical expenses incurred.
RELEASE OF LIABILITY I hereby release, waive, and discharge AV Dream, its coaches, volunteers, staff, organizers, and affiliates from any and all claims, demands, or causes of action arising out of or related to my child’s participation, including injuries or damages, whether caused by negligence or otherwise.
NO REFUND POLICYAll registration fees and monthly payments for the AV Dream Dream Girls Cheer Program are NON-REFUNDABLE.By registering your child, you acknowledge and agree to the following:No refunds will be issued for registration fees or monthly payments.No refunds will be provided for missed practices, games, or events.No refunds will be issued if a participant voluntarily withdraws from the program.Registration fees help cover program operating expenses, staffing, insurance, and facility costs.We appreciate your understanding and support of AV Dream and our commitment to providing a positive experience for all participants.By completing registration and submitting payment, you acknowledge and agree to this policy.
