Children/Youth Event Travel Release Form

I, the undersigned, am the parent or legal guardian of the participant named above and hereby give permission for my child to participate in any and all activities associated with the above-mentioned event organized by Twelfth Avenue Baptist Church. This includes riding in any vehicle designated by Twelfth Avenue Baptist Church, its employees and adult volunteers while participating in and traveling to and from these events. I acknowledge that participation in this event may involve risks including, but not limited to, travel risks, physical activity, injury, illness, and unforeseeable incidents. I voluntarily assume all such risks on behalf of my son/daughter.In consideration of my child being allowed to participate, I hereby release, discharge, and hold harmless Twelfth Avenue Baptist Church, its pastors, employees, agents, and volunteers from any and all claims of liability, loss, or injury, including death, that may be sustained by my child while participating in this event or traveling to/from the event, whether caused by negligence or otherwise.
