Midpoint Youth Lock-In! August 7-8 5:30pm-9:00am

This is a night designed just for you, a chance to laugh, make new friends, grow closer with the students around you, and deepen your realtionship with Jesus. 

We'll have fun games, delicious food, movies,worship, a meaningful devotion, and small groups where you connect, ask questions, and be encouraged in your walk with Christ. Whether you've been following Jesus for years of you're just beginning to explore your faith, this night is for you!

Come ready for a night full of joy, encouragement, and memories you'll never forget. We can't wait to spend the night with you as we grow together in friendship and faith!

 

In the event of an accident, illness, or medical emergency, and I cannot be reached immediately, I authorize MidPoint Church staff, volunteers, or emergency personnel to obtain necessary emergency medical treatment for my child.

I understand that every reasonable effort will be made to contact me before treatment whenever possible.

I accept responsibility for any medical expenses incurred on behalf of my child.

I give permission for my child to participate in the MidPoint Church Girls' Lock-In, including all scheduled activities occurring on church property during the event.

I understand this is an overnight event and give permission for my child to remain at MidPoint Church for the duration of the event unless checked out by an authorized adult.

I understand that participation in church activities involves certain inherent risks, including but not limited to slips, falls, physical activity, interactions with other participants, accidents, weather-related conditions, and other unforeseen circumstances.

I voluntarily permit my child to participate and knowingly assume these ordinary risks associated with participation.

To the fullest extent permitted by North Carolina law, I release and hold harmless MidPoint Church and its pastors, employees, volunteers, officers, directors, and representatives from claims arising out of ordinary risks associated with my child's participation in this event.

I understand that this release does not apply to conduct that cannot legally be released under applicable law.

I understand that participants are expected to:

• Treat others with kindness and respect.

• Follow all instructions given by church staff and volunteers.

• Remain in designated areas unless supervised.

• Respect church property.

• Refrain from behavior that creates an unsafe environment.

I understand that if my child demonstrates unsafe, disruptive, or inappropriate behavior, I may be contacted and required to pick up my child before the event concludes.

MidPoint Church occasionally photographs and records church events for ministry purposes, including social media, promotional materials, printed publications, and the church website.

I understand that MidPoint Church utilizes security cameras throughout portions of its campus for the safety and security of guests, staff, volunteers, and participants.

I understand that my child may be recorded while on church property and that any security footage remains the property of MidPoint Church and may be used for security, safety, operational, or legal purposes.

By signing below, I acknowledge that:

• I have read and understand this entire form.

• The information I have provided is accurate to the best of my knowledge.

• I voluntarily grant permission for my child to participate.

• I understand the nature of the activities involved.

• I understand that MidPoint Church will make reasonable efforts to provide a safe environment but cannot eliminate all risks associated with participation.
