1. Acknowledgement of Risks:
I understand that participation in the Winter Break Camp involves certain risks and dangers, including but not limited to, those associated with transportation, activities, and interactions with other participants. These risks include, but are not limited to, bodily injury, property damage, illness, and in extreme cases, death. I acknowledge that it is my responsibility to ensure my child is aware of and follows all safety instructions provided by the camp staff.
2. Health and Medical Authorization:
I hereby certify that my child is physically fit to participate in the Winter Break Camp activities. In the event of an injury or medical emergency, I authorize D.B.G.E.M. - Developing Boys Into Gainfully Employed Men, Inc. staff to seek medical treatment for my child. I also agree to be responsible for any and all medical and related bills that may be incurred on behalf of my child.
3. Waiver of Liability:
In consideration of my child being permitted to participate in the Winter Break Camp, I, on behalf of myself, my child, our heirs, executors, administrators, and assigns, hereby waive, release, and forever discharge both D.B.G.E.M. - Developing Boys Into Gainfully Employed Men, Inc., and Paulding Northwest Atlanta Airport, the and their officers, its officers, directors, employees, agents, and volunteers from any and all claims, demands, and causes of action, which may arise out of or in connection with my child's participation in the Winter Break Camp, including those which arise out of or result from any transportation and/or entertainment provided by D.B.G.E.M. - Developing Boys Into Gainfully Employed Men, Inc.
4. Indemnification:
I agree to indemnify and hold harmless both D.B.G.E.M. - Developing Boys Into Gainfully Employed Men, Inc., and Paulding Northwest Atlanta Airport, its officers, directors, employees, agents, and volunteers from and against any and all liabilities, claims, demands, damages, losses, or expenses, including reasonable attorney's fees and costs, arising out of or related to my child's participation in the Winter Break Camp.
5. Transportation Conditions:
I acknowledge that transportation to and from camp activities may be provided by D.B.G.E.M. - Developing Boys Into Gainfully Employed Men, Inc. or its contracted transportation service providers. I understand that while every effort will be made to ensure safe transportation, accidents can occur. I release D.B.G.E.M. - Developing Boys Into Gainfully Employed Men, Inc. and Paulding Northwest Atlanta Airportand its contracted transportation service provider from any liability arising from accidents, injuries, or damages that may occur during transportation to and from camp activities.
6. Media Release:
I grant permission for D.B.G.E.M. - Developing Boys into Gainfully Employed Men, Inc. and Paulding Northwest Atlanta Airport to use photographs, videos, or other media of my child taken during the Summer Camp for promotional purposes, including but not limited to social media, websites, and marketing materials.
7. Emergency Contact Information:
I agree to provide accurate emergency contact information for myself and any alternate contacts during the Winter Break Camp. I understand that it is my responsibility to keep this information up to date and inform D.B.G.E.M. -Developing Boys Into Gainfully Employed Men, Inc. of any changes. When the student is delivered and picked up, sign in and check out are required.
I have read this Parental Liability Waiver and fully understand its terms. I acknowledge that I am voluntarily giving up certain legal rights that I might otherwise have, and I agree to be bound by its terms. There are no refunds, students are required to participate in the activities.
