CONSENT
I/We, the parent(s)/guardian of the above-named student, who is a candidate for participation in this 2022 Olentangy Berlin Sports Summer Camp, hereby give(s) approval and consent for his/her participation in any and all activities of said program. I/We do hereby release, acquit and forever discharge and agree to indemnify and save harmless the Olentangy Berlin Athletic Boosters, its coaches and supervisors, and all other persons assisting in the conduct of said activities and from all actions, causes of action, claims, or demands of whatever nature arising of injuries to, or death of, the above-named student as a result of all activities of said program.
