Thank you for your interest in "When Kids IMAGINE" summer camp. This camp helps youth grades 2nd-8th learn through fun academic, Young Authors, Life Skills, Healthy & Fit, Kidpreneur programs, unlocking possibilites kids never imagined. Please take a moment and review the following registration questionaire as we look forward to seeing your child participate this upcoming summer. Keep in mind, we have limited spots, so please ensure your registration is complete in order to successfuly make it to our camp attendance list.

I.M.A.G.I.N.E Mentoring brings together resources in the community with students in their schools  so they can learn, grow, and succeed in school and in life. I.M.A.G.I.N.E Mentoring, and any affiliated production companies, vendors, contractors, and  representatives are authorized to photograph, interview and/or film:

This agreement permits the I.M.A.G.I.N.E Mentoring (IMAGINE) and photographers, film companies  and interviewers working with them, to use his/her name and image obtained via any interview, format  of motion picture or still photography means (including, but not limited to, use of negatives, prints, film  

stock or video tape derived from photographic sessions). This individual’s image may be used without  restriction in a way that is deemed necessary or appropriate, including, but not limited to, spreading  information for educational, public relations, community affairs, print marketing, online media (such as  Facebook, Twitter, Instagram, Facebook Business Suite, Tik Tok, Snap Chat or other social media outlets  involving youth) with purposes to benefit I.M.A.G.I.N.E. 

This individual or the parent/guardian of this minor/student enters into this agreement to assist  I.M.A.G.I.N.E Mentoring to educate the public and promote the program. This individual or the  parent/guardian of this minor/student hereby waives any right to compensation for such uses. The  individual or his/her guardian and his/her successors hereby hold I.M.A.G.I.N.E, the owner/founder of the  organization, team members, facilitators and its affiliated vendors, companies, and/or representatives,  harmless from and against any claim for compensation of any kind or any other claims. 

The individual signing this form verifies the s/he is legally authorized to sign on behalf of himself/herself  or the minor student. The individual acknowledges that providing this authorization is completely  voluntary and that there are no negative consequences for students or parents who choose not to  participate. We ask that the student notifies that staff member that they do not consent to  video/photos avoid being involved any group marketing videos/photos. 

Students who register for our Summer Discovery programming must commit to attending programming throughout the summer.  Summer Discovery is not a drop-in program, and it's important for students to attend daily to receive the full benefits of the program.  

Please note:  Along with assessments, your child will be asked to complete a post-survey after the program so that we can evaluate any program0related successes and challenges this year in order to continue to improge Summer Discovery to best serve all students and families.  No further action is requested to provide for your child to participate.  If you would prefer that your child not to participate, please notify us via email imagineevents0@gmail.com confirming you would like to opt-out of student surveys.
